💗 BEFORE STARTING HRT, DO YOU NEED A MAMMOGRAM?
Breast screening is an important part of menopause care—but a mammogram is not simply a “permission test” for HRT.
Before starting hormone replacement therapy, we should review:
🔹 Whether your routine mammogram is up to date for your age and country
🔹 Your breast density
🔹 Your personal and family history
🔹 Previous biopsies or abnormal breast imaging
🔹 Any new breast lump, ni**le discharge, skin change or persistent focal pain
If you have a new breast symptom or are overdue for screening, this should be assessed promptly. But for an asymptomatic woman whose screening is current, HRT does not automatically require extra mammograms outside the recommended schedule.
💗 DO YOU KNOW YOUR BREAST DENSITY?
Breast density is usually classified as:
A — Almost entirely fatty
B — Scattered areas of density
C — Heterogeneously dense
D — Extremely dense
Dense breasts are common and do NOT mean that you have breast cancer. However, density matters because:
🔹 Dense tissue can make cancer more difficult to detect on a mammogram.
🔹 Extremely dense breasts are independently associated with a higher breast-cancer risk.
🔹 HRT—particularly combined estrogen and progestogen therapy—may increase breast density in some women and occasionally lead to additional imaging or callbacks.
Dense breasts are not, by themselves, a contraindication to HRT. The decision should be individualized—not based on fear.
For women aged 50–70 with extremely dense breasts, the European Society of Breast Imaging recommends discussing additional MRI screening every 2–4 years, preferably every 2–3 years. If MRI is unavailable or unsuitable, ultrasound in addition to mammography may sometimes be considered.
The right question is not simply:
“Can I take HRT?”
It is:
“What are my individual benefits and risks, and what breast-screening plan is appropriate for me?”
Know your breast density.
Know your personal risk.
Keep your screening up to date.
Personalize both your HRT and your breast care.
