Apologies for the underwhelming photography. It's a bit suble. But we don't actually see this condition all that commonly even though it has pride of place in Kanski and most Optometry students would have seen it in texts. Patient presents with over 12 months of photophobia, burining and ocular discomfort. Both eyes. Diagnosed with dry eye but limited symptomatic improvement with lubricants. Diagnosis? Maybe we don't look for it enough?

Depending on the method employed, marking DMEK tissue can be cumbersome and require expensive trephines. Performing a triangular marking is quick and utilises the keratome already in your DMEK surgical tray. Note, when harvesting the donor, the long edge of the triangle is cut to be conintuous with the graft edge counter clockwise. When checking the graft orientation this is reversed. When inserted, if the long side of the triangle is continuous clockwise with the graft edge then the graft orientation is correct!
