Moments like these remind us why collaborative supports matter so much 💙
These two amazing young boys recently attended their school colour run and absolutely thrived throughout the event with no escalations. One of the boys has also recently received a school award recognising his positive behaviour across the term — an incredible achievement and reflection of the hard work, growth, and support surrounding him.
Behind moments like this is a village of people working together. Ongoing collaboration between family, school staff, Social Work, Behaviour Support, Child Safety, Paediatric supports, advocates, allied health, community services, and NDIS supports has helped create consistency, stability, and positive outcomes for both boys and their family as a whole.
These outcomes do not happen overnight, and they are never achieved by one system alone. Real progress often comes from Health, Education, Child Services, community supports, and the NDIS all working together to ensure families feel supported both at home and within the community.
Watching these boys participate, engage, and succeed in their school community has been incredibly rewarding and a reminder of what can happen when the right people come together around a family 💙

Hi everyone — we thought it was time to properly introduce ourselves. 💕

We are Melissa and Tracy, the directors behind Holt For Moore Support (HFMS).

HFMS was created from genuine passion, lived experience, and a deep understanding of how important the right support truly is. We built this business because we wanted to create something different — support that is personal, compassionate, and truly centred around the individual.

For us, this has never just been about providing a service. It’s about building real connections, advocating fiercely for the people we support, and creating a safe, supportive environment where participants and families feel heard, valued, and genuinely cared for.

We know how overwhelming systems can be. We know how important it is to have people in your corner who actually listen and want the best outcomes for your loved ones. That is the heart behind everything we do at HFMS.

As we continue to grow into Support Coordination, In-Home Supports, and Community Access, our vision remains the same — providing meaningful support with compassion, understanding, and heart.

✨ We just wanted to take a moment to say how incredibly proud we are of Bree! ✨

Over the past little while, Bree has been stepping outside of her comfort zone and making huge progress exploring the community and trying new experiences. 💜

What may seem small to others can actually be massive steps for someone pushing through anxiety, uncertainty, or fear — and Bree has absolutely been making leaps and bounds. From getting out into the community more, building confidence, trying new things, and embracing new experiences, she has shown so much courage and determination.

Seeing the enjoyment, happiness, and confidence grow with every outing has been amazing to watch. 🌸

Bree, you should be so proud of yourself. The growth, resilience, and strength you continue to show does not go unnoticed, and we cannot wait to see what other amazing experiences are ahead for you.✨
