A little overdue, but… this is me 😊
I’m Effie, the person behind Together Pathways, a mum of three and someone whose connection to disability began long before it became my work.
My brother has an intellectual disability and sustained a brain injury following an accident ten years ago. My older sister also sustained a brain injury at birth.
Growing up in a Greek family with parents who don’t speak English shaped my understanding of how difficult it can be to navigate services, understand information and make sure your family’s needs are heard.
As a family, we’ve experienced care that fell short of what our loved ones needed. Those experiences stayed with me and are a big part of why I care so much about listening, asking questions and following things through.
I know every family’s experience is different. Mine is what I bring with me when I sit down and listen to yours.
I support people with a wide range of disabilities, including intellectual disability, acquired brain injury and autism.
That might mean helping someone understand their options and where to start, finding supports that are a better fit, or helping a family navigate a difficult situation or change in needs.
I work alongside support workers, care agencies, day programs, therapists, schools and other services to help everyone communicate and work towards what matters to the person.
A big part of my role is noticing what may be missing, asking questions, following things up and advocating when someone doesn’t feel heard. When more support is needed, I also help gather the reports and evidence required to request additional funding.
Sometimes, it’s simply being the person someone can call and say, “I’m overwhelmed. Can you help me work through this?”
I love getting to know the people I support, building genuine relationships and seeing things begin to come together. Seeing someone feel heard, have more choice or take a step towards something that matters to them is why I love this work.
So, this is me and a little of the story behind Together Pathways 🤍

Ending the day with a reminder that this work is about so much more than plans, budgets and referrals
It’s about people feeling heard, families feeling supported, and making sure the right supports come together at the right time
Some days the progress is big
Other days it’s a phone call, an introduction, a follow-up, or simply being there to help someone navigate what can sometimes feel like a really overwhelming system
Those little things matter too 🤍
That’s what meaningful Support Coordination looks like to us at Together Pathways

Understanding Core Supports
Core Supports are often the largest part of an NDIS plan, but many participants and families aren't always sure what they can be used for
Core Supports are designed to help with everyday activities and increase independenceDepending on your plan, this may include support with personal care, community access, assistance around the home, transport, consumables, or support to participate in social and community activities
One of the benefits of Core Supports is that they can often provide flexibility, allowing funding to be used where it is needed most to help achieve your goals
Understanding how your Core Supports funding can be used can help you get the most out of your NDIS plan and ensure the supports you access are working towards what matters most to you.
If you're unsure about your Core Supports funding or how it can be used, speaking with your Support Coordinator can be a great place to start

You're not alone
I know firsthand how confusing and overwhelming the NDIS can feel
Between funding, providers, service agreements, reviews and knowing where to start, it's easy to feel lost
That's where Support Coordination can help
At Together Pathways, I work alongside participants and families to help navigate the NDIS, connect with the right supports and make the most of their plan

Today I attended an NDIS access meeting for a young person and their family
Listening to their story was a reminder of how much happens behind closed doors that most people never see
When people think about disability, they often see the diagnosis or the challenges the individual faces
What they don't always see is the impact on the whole family
The family who no longer attends many social events because it's just too difficult
The parents who are constantly planning ahead, trying to avoid situations that may become overwhelm in
The siblings who miss out on things or have their own lives affected by circumstances outside of their control
They don't see the sleepless nights, the stress, the worry, or the endless appointments, reports and phone calls
They don't see the strength it takes to keep showing up every day and doing the best they can
Today's meeting was a powerful reminder that behind every NDIS application and every request for support is a family with a story
A family doing their best
A family who deserves to be heard, understood and supported
It's moments like these that remind me why I do what I do

Today I had the privilege of supporting a family through the NDIS access process for their 15 year old son ♥️
For many families, the NDIS can feel overwhelming Understanding what evidence is needed, completing paperwork, and navigating the process can be stressful, especially when you're already supporting a child with additional needs
One of the most rewarding parts of my role as a Support Coordinator is helping families understand their options, advocate for their loved ones, and feel confident throughout the journey
Every family's circumstances are unique, but no family should have to navigate the system alone
I'm passionate about helping participants and families access the supports they need to build capacity, increase independence, and work towards their goals Guiding your NDIS journey with care and clarity

Behind every participant is often a whole team of people working together 🤍
When providers communicate well and work together properly, it can make such a big difference for both participants and families
It helps create more consistent supports, clearer communication, and better overall outcomes
Support coordination is about so much more than appointments and services, it’s about making sure people feel supported, understood, and not left to navigate everything on their own
