You will assist with daily living, community access, transport, and skill-building to help clients achieve independence and meaningful outcomes.

Key Responsibilities:

Provide active support tailored to each client’s needs

Assist with personal care and daily living activities

Support clients with social and community participation

Transport clients safely to appointments and activities

Assist with documentation and shift reporting

Maintain professional communication with families and support teams

Requirements:

Relevant experience or qualifications

NDIS Worker Screening Check

Working With Children Check (WWCC)

Current First Aid certification

Police Check

Manual Handling Training

Driver’s licence and reliable car (mandatory)

The ideal candidate will have strong organisational skills, knowledge of the NDIS framework, and a passion for empowering individuals to achieve their goals.

Key Responsibilities:

Assist participants to understand and implement their NDIS plans

Coordinate supports and link clients with appropriate providers

Build and maintain strong relationships with participants and stakeholders

Key Responsibilities:

Conduct assessments and deliver therapeutic interventions

Develop individualised treatment plans for clients

Work collaboratively with families, carers, and support teams

Prepare reports, progress notes, and NDIS-required documentation

Provide centre-based, community-based, or in-home services

Contribute to a multidisciplinary environment

Requirements:

Relevant degree and professional registration

Clinical experience within disability or community settings

Strong communication and report-writing skills

Commitment to person-centred and evidence-based practice
