The NDIS is currently seeking feedback on the future Support Coordination and Connection Service, including how it should work, how people access it, quality and safeguarding, and what good support should look like.

Ask one important question:
If one provider disappeared tomorrow, what would break? ⚠️🧩

Would the participant lose access to personal care, meals, medication support, transport, appointments, community access, overnight assistance, or important information?

Depending too heavily on one provider can create vulnerability—even when that provider is currently performing well.

A continuity plan may include:
• Backup providers
• Updated contact details
• Shared documentation
• Clear emergency arrangements
• An agreed response when regular support is unavailable

Planning for continuity is not expecting failure. It is protecting the participant’s safety, stability, and everyday life. 🛡️

Would the participant lose access to personal care, meals, medication support, transport, appointments, community access, overnight assistance, or important information?

Not sure whether Level 2 or Level 3 support coordination is appropriate? 🧭🤔

Start with the situation—not the label.

Level 2 Support Coordination generally helps participants put supports in place, connect with services, manage tasks, and build their ability to coordinate supports.

Level 3 Specialist Support Coordination provides a higher level of assistance for participants with complex needs who require specialist support.

Consider:
• How many services and systems are involved?
• Are there immediate or ongoing risks?
• Is housing unstable?
• Are services repeatedly breaking down?
• Are significant barriers preventing the plan from being used?

The appropriate pathway depends on the participant’s individual needs, circumstances, and approved funding. ✅

A plan that worked before may no longer meet the participant’s needs after a significant change. 🔄📝

Changes may involve:
• Housing
• Health
• Safety
• Informal support
• Providers
• Functional capacity
• Daily support requirements

Record what changed, when it happened, how daily life has been affected, and which current supports are no longer working.
A change in circumstances does not always require a new plan. However, when the current plan no longer meets the participant’s disability-related support needs, they may need to ask the NDIA about changing the plan and provide relevant evidence.

Ask for help before the situation reaches crisis point. 🤝
