👉 এটা একটি advanced অপারেশন যেখানে screw + plate দিয়ে হাড় শক্তভাবে বসানো হয়

একজন রোগী tibia ও fibula shaft fracture নিয়ে আসেন — তীব্র ব্যথা, হাঁটতে অক্ষম, এবং পা ফুলে গিয়েছিল।

এই ধরনের fracture ঠিকমতো চিকিৎসা না হলে ভুলভাবে জোড়া লাগা, দীর্ঘদিন ব্যথা, এমনকি হাঁটার সমস্যাও হতে পারে।

এই রোগীর জন্য করা হয়েছে Closed Reduction and Internal Fixation with Tibial Interlocking Nail (IM Nail) —
অর্থাৎ বড় করে কাটা ছাড়াই ছোট incision দিয়ে হাড় ঠিক alignment এ এনে nail প্রবেশ করানো হয়েছে।

✅ কম soft tissue damage
✅ fracture site কম expose করতে হয়

A patient presented with fracture shaft of tibia and fibula with severe pain, swelling, and inability to walk.

For this case, we performed Closed Reduction and Internal Fixation with Tibial Interlocking Nail — a minimally invasive technique where the fracture is aligned and fixed without opening the fracture site extensively.

Fracture Humerus: When is Operation Needed? | হিউমেরাস হাড় ভাঙলে কখন অপারেশন লাগে?

🦴 Humerus fracture = fracture of the upper arm bone (shoulder থেকে elbow পর্যন্ত লম্বা হাড়)।

সব humerus fracture-এ অপারেশন লাগে না। অনেক ক্ষেত্রে plaster/brace দিয়েও ভালো হয়। তবে কিছু নির্দিষ্ট অবস্থায় surgery দরকার হয়।

Indications of Operation | অপারেশনের প্রয়োজন হয় যেসব ক্ষেত্রে
1. Open fracture (খোলা ভাঙা)
হাড় ভেঙে বাইরে বের হয়ে এলে infection-এর ঝুঁকি বেশি থাকে, তাই জরুরি অপারেশন প্রয়োজন।

2. Displaced fracture (হাড় বেশি সরে গেলে)
যদি bone alignment ঠিক না থাকে এবং plaster দিয়ে ঠিক রাখা সম্ভব না হয়।

3. Comminuted fracture (হাড় অনেক টুকরো হলে)
Multiple fragments থাকলে surgery often gives better stability.

4. Fracture with nerve injury
বিশেষ করে Radial nerve palsy থাকলে evaluation দরকার, কিছু ক্ষেত্রে operation লাগে।

5. Vascular injury
রক্তনালীর ক্ষতি হলে emergency surgery লাগতে পারে।

6. Pathological fracture
হাড় দুর্বল (tumor/infection/osteoporosis) থাকার কারণে fracture হলে surgery may be needed.

7. Nonunion / Malunion
হাড় না জোড়া লাগলে বা বেঁকে জোড়া লাগলে।

8. Polytrauma patient
একাধিক আঘাত থাকলে early fixation helps early mobilization.

9. Intra-articular fracture
Shoulder joint / elbow joint পর্যন্ত fracture extend করলে।

Options of Operation | অপারেশনের ধরন

1. Plate and Screw Fixation (ORIF)
➡️ হাড় সোজা করে plate & screw দিয়ে fixation করা হয়।
Best for:
Shaft fracture
Distal humerus fracture
Some proximal fractures

2. Intramedullary Interlocking Nail (IM Nail)
➡️ হাড়ের ভেতর rod/nail ঢুকিয়ে fixation করা হয়।
Best for:
Shaft fracture
Segmental fracture
Some pathological fractures

3. External Fixator
➡️ বাইরে থেকে rod/frame দিয়ে temporary or definitive fixation।
Used in:
Open fracture
Severe soft tissue injury
Damage control surgery

4. Shoulder Replacement / Arthroplasty
➡️ Elderly patients with severe proximal humerus fracture where fixation is difficult.
Examples:
Hemiarthroplasty
Reverse shoulder arthroplasty

5. Bone Grafting + Fixation
যদি nonunion হয় বা bone loss থাকে।

Non-operative treatment কখন সম্ভব?
✔ Minimally displaced fracture
✔ Functional brace works well
✔ Patient medically unfit for surgery

🦴 “একটা সফল অপারেশন শুধু surgeon-এর skill না — OT-তে sterility maintain করাও equally lifesaving!”

Total Hip Replacement / যেকোনো major orthopedic surgery-তে শুধু implant বসানোই কাজ না — infection prevent করা সবচেয়ে বড় challenge.
একটি Total Hip Replacement surgery-তে যদি proper sterility maintain না করা হয়, তাহলে implant infection হতে পারে, যা রোগীর জন্য অত্যন্ত কষ্টকর এবং কখনও কখনও implant removal পর্যন্ত প্রয়োজন হতে পারে.

আমাদের Orthopedic OT-তে আমরা নিশ্চিত করি:

Giant Cell Tumor of Soft Tissue (Second Toe)
একজন রোগী দীর্ঘদিন ধরে পায়ের ২য় আঙুলে ছোট একটা গুটি লক্ষ্য করছিলেন।
প্রথমে তেমন গুরুত্ব দেননি… কিন্তু ধীরে ধীরে এটি বড় হতে থাকে 😟

MRI তে দেখা গেল—
👉 টিউমারটি আঙুলের চারপাশেই ছড়িয়ে গেছে (circumferential involvement)
এই অবস্থায় শুধু গুটি কেটে ফেললে হতো না ❌
কারণ এতে পুনরায় ফিরে আসার (recurrence) ঝুঁকি খুব বেশি

👉 তাই করা হলো Ray Amputation (2nd toe) — পুরো আক্রান্ত অংশটি নিরাপদভাবে অপসারণ করা হয়েছে

🔬 অপারেশনের পর কাটা অংশটি পাঠানো হয়েছে বায়োপসি (Histopathology) এর জন্য যাতে নিশ্চিত হওয়া যায় এটি Giant Cell Tumor of Soft Tissue কিনা এবং সম্পূর্ণভাবে অপসারণ হয়েছে কিনা

🦴 Challenging Tibial Shaft Fracture Case

It was a case of a closed comminuted segmental fracture of the tibial shaft in a young man.

I performed closed reduction and internal fixation using an IMIL (Intramedullary Interlocking) nail.

The procedure was particularly challenging because the spiral fracture line extended up to the medial malleolus, making it difficult to maintain proper reduction during nailing.

Despite the complexity, intramedullary nailing remains the best treatment option for this type of fracture, offering better stability, minimal soft tissue disruption, and faster recovery compared to plating.

Careful technique and intraoperative precision were key to achieving a satisfactory outcome.

🦴 এটি ছিল একজন young patient-এর closed comminuted segmental fracture of the tibial shaft।

আমি এই case-এ করেছি closed reduction and internal fixation with IMIL (Intramedullary Interlocking) nail।

👉 Challenge কোথায় ছিল?
Fracture pattern ছিল খুব complex — spiral fracture line extend করে medial malleolus পর্যন্ত চলে গিয়েছিল, যার কারণে reduction maintain করা এবং proper nailing করা ছিল quite difficult।

👉 তারপরও কেন nail বেছে নিলাম?

ইলিজারভ এক্সটার্নাল ফিক্সেটর (Ilizarov) — ভাঙা হাড় জোড়া লাগানো ও হাড় লম্বা করার আধুনিক সমাধান

পায়ের টিবিয়া (Tibia) হাড় ভেঙে গেলে অনেক সময় সাধারণ প্লেট-স্ক্রু বা নেল দিয়ে চিকিৎসা সম্ভব হয় না—বিশেষ করে যদি হাড়ে সংক্রমণ থাকে, নন-ইউনিয়ন হয়, বা হাড়ের দৈর্ঘ্য কমে যায়। এই জটিল সমস্যার কার্যকর সমাধান হলো Ilizarov External Fixator।

👉 এই পদ্ধতিতে হাড়ের বাইরে একটি রিং (Ring) ও তারের (Wire) মাধ্যমে হাড়কে স্থির রাখা হয়, যা ধীরে ধীরে হাড়কে জোড়া লাগাতে সাহায্য করে।

✅ কীভাবে কাজ করে?
✔️ হাড়ের ভাঙা অংশকে স্থির রেখে ধীরে ধীরে নতুন হাড় তৈরি (Bone regeneration) করে
✔️ নন-ইউনিয়ন বা ইনফেকটেড ফ্র্যাকচারেও সফল
✔️ হাড় ছোট হয়ে গেলে ধাপে ধাপে লম্বা করা যায় (Bone lengthening)
