চোখে আঘাতের পর ৭ দিন অপেক্ষা—দৃষ্টির জন্য কতটা বিপজ্জনক???

চোখে আঘাত লেগেছে, কিন্তু তখন তো খুব বেশি সমস্যা ছিল না—তাই ডাক্তার দেখাইনি!

এই সামান্য অবহেলাই কখনো কখনো চোখের জন্য বড় বিপদ ডেকে আনতে পারে।
এই রোগী ৭ দিন আগে ডান চোখে আঘাত পেয়েছিলেন।
আজ এসেছেন তীব্র চোখের ব্যথা, দৃষ্টিশক্তি কমে যাওয়া নিয়ে।

পরীক্ষায় দেখা গেল—
🔴 আঘাতের কারণে লেন্সের সামনের দিকে সাবলাক্সেশন (traumatic anterior lens subluxation)
🔴 এর ফলে secondary glaucoma ,
চোখের ভেতরের চাপ বিপজ্জনকভাবে বেড়ে গেছে।

আঘাতের পর লেন্সের অবস্থান পরিবর্তিত হলে সেটি চোখের drainage pathway বাধাগ্রস্ত করে হঠাৎ IOP বেড়ে যেতে পারে এবং secondary glaucoma তৈরি করতে পারে।এমন অবস্থায় দ্রুত ophthalmic assessment ও প্রয়োজন অনুযায়ী জরুরি চিকিৎসা/সার্জারি প্রয়োজন হতে পারে।

তাই চোখে আঘাত লাগার পর “দেখি, ঠিক হয়ে যায় কি না”—এই অপেক্ষা করবেন না।

বিশেষ করে যদি থাকে—
• চোখে ব্যথা
• দৃষ্টি ঝাপসা/কমে যাওয়া
• আলো সহ্য না হওয়া
• চোখ লাল হওয়া
• pupil-এর আকৃতি পরিবর্তন
• আঘাতের পর হঠাৎ মাথাব্যথা বা বমি

দেরি না করে একজন ophthalmologist-এর কাছে জরুরি চোখ পরীক্ষা করান।

একটি ছোট্ট লেজার—দৃষ্টিশক্তি বাঁচানোর বড় সুযোগ!

এই রোগীর Angle-Closure Glaucoma ধরা পড়ে—যেখানে চোখের পানি বের হওয়ার স্বাভাবিক পথ বন্ধ বা সংকুচিত হয়ে চোখের চাপ বিপজ্জনকভাবে বেড়ে যেতে পারে।

এই রোগীর ক্ষেত্রে আমরা Laser Peripheral Iridotomy (Laser PI) করেছি। চোখের মণির চারপাশের আইরিসে একটি ক্ষুদ্র ছিদ্র তৈরি করে চোখের ভেতরের তরল চলাচলের পথ সহজ করা হয়েছে।

কিন্তু মনে রাখবেন—Angle-Closure Glaucoma কখনো কখনো হঠাৎ করেই দৃষ্টিশক্তির জন্য মারাত্মক হুমকি হয়ে উঠতে পারে।

যেসব লক্ষণকে অবহেলা করবেন না:

⚠️ তীব্র চোখব্যথা
⚠️ প্রচণ্ড মাথাব্যথা
⚠️ হঠাৎ ঝাপসা দেখা
⚠️ আলোর চারপাশে রঙিন বৃত্ত দেখা
⚠️ চোখ লাল হয়ে যাওয়া
⚠️ বমি বমি ভাব

মাথাব্যথা” বা “চোখ ঝাপসা” অবহেলা করবেন না।

She came to my chamber with her mother and her 15-year-old sister… but she was already carrying a lifetime of darkness.

Today, a 23-year-old girl came to me.
Her vision?

Both eyes: PL only.

Her right eye was already phthisical.

Her left eye had advanced glaucoma,with uncontrolled IOP despite maximum available anti-glaucoma medications.

I looked at her mother and her 15-year-old sister sitting beside her.

Then her mother said something that broke my heart:

I can’t bear this burden anymore.
Not because she doesn't love her daughter.

But because she has been watching her daughter lose her vision, day after day, while knowing how little she can do.

And I sat in my chamber thinking—
What dreams did this girl have at 23?

A career?
A family?
Travelling?
A normal independent life?

Today, her family is not talking about dreams.
They are fighting to preserve whatever remains.

As glaucoma specialists, we often talk about IOP, optic nerve damage, medications, surgery and target pressure.

But behind every number is a human being.
Behind every advanced glaucoma patient,there is a family😔😔

At 28 she is not fighting for a career.
She is fighting for her VISION…

A 28 year-old girl.
Right eye: Hand Movement
Left eye: 3/60

She has advanced glaucoma in both eyes and despite maximum medications, her IOP remains uncontrolled.

Now, every day is a fight to preserve the little vision she has left.

Think about it for a moment…

At 28 most of us are running after our dreams.

Planning our careers.
Making future plans.
Travelling.
Falling in love.
Building a life.

But where is this girl?

She is fighting to see the life she is supposed to live.
And the most heartbreaking part?
She didn't have any obvious eye symptoms.

She did mention having headaches in the past—but she ignored them.

when headache occurs together with eye pain, blurred vision, halos around lights, redness, nausea or vomiting, it can be a warning of a sudden rise in eye pressure.

So please don't ignore persistent or unusual headaches when they are associated with visual or eye symptoms.

Today, this young girl is not asking for perfect vision.
She is fighting to keep the vision she still has.
And that is why glaucoma screening matters.

Because your eyes may be carrying your dreams…
while glaucoma is quietly trying to take them away.

The most heartbreaking consultation wasn't about one blind patient… it was about two.

Today, a 60-year-old man came to my chamber.

His right eye could only perceive light. His left eye had no perception of light (NPL)

He wasn't alone.

He came with his elderly mother... who was also blind from advanced glaucoma.She had no perception of light in both eyes.

Neither of them could walk into my chamber without help.

His wife gently guided both of them, becoming the eyes for two generations of her family.

After examining them, I had to say ..
I'm sorry... we cannot bring the vision back.

The room fell silent.
Today, I didn't just see two blind patients.

I saw the devastating impact of a disease that could have been detected earlier.
Please don't wait for symptoms.
If you are over 40 years old, have a family history of glaucoma, diabetes, high eye pressure, or use steroid medications, have your eyes examined regularly.

There are some conversations that stay long after the clinic is over.

Today, a 65-year-old man sat in front of me with tears in his eyes.

A year ago, he could see well enough to live independently. Despite undergoing glaucoma and cataract surgery, relentless glaucoma progression, compounded by a central retinal vein occlusion, gradually stole his sight. Today, one eye has only light perception, and the other has no light perception.

He looked at me and said,

Doctor, I just want to see the world again. Please... do anything to bring back my vision."

As a glaucoma specialist, those words are heartbreaking.

Glaucoma blindness is irreversible. Once the optic nerve is permanently damaged, we can only try to preserve the vision that remains—we cannot restore what has already been lost.

His tears reminded me of something we often forget.

We complain about traffic.
We complain about work.
We complain about small inconveniences.

Yet millions would give everything just to see the faces of their loved ones, the colours of the sky, or the sunrise one more time.

If you can read this post today, pause for a moment and be grateful for your eyesight.

"Doctor, can I ever see again?"

That was the question a 17-year-old girl asked me today.

She has juvenile open-angle glaucoma.

Right eye vision: 6/60
Left eye vision: Hand movements only

Because of her father's financial hardship, she stopped using her glaucoma eye drops. Month after month, the silent disease continued to damage her optic nerves. By the time she returned, her vision had been fading day by day.

I had to tell her the hardest truth:

The vision already lost from glaucoma cannot be restored.
Glaucoma is called the "silent thief of sight" for a reason. It steals vision slowly, often without pain or warning. Missing medications, skipping follow-up visits, or stopping treatment—even because of financial difficulties can lead to irreversible blindness.

To everyone reading this:

✅ Never stop glaucoma medication without consulting your eye doctor.
✅ If treatment becomes financially difficult, discuss it with your ophthalmologist. There may be affordable alternatives.
✅ Early diagnosis and regular treatment can preserve sight for a lifetime.

Glaucoma doesn't ask your age..

This 18-year-old developed steroid-induced glaucoma after prolonged use of steroid eye drops. Despite treatment with medications, his eye pressure remained uncontrolled, and he required trabeculectomy in both eyes to protect his remaining vision.
This case is a reminder that:
✅ Glaucoma can affect young people.
✅ Steroid eye drops should never be used without proper medical supervision.
✅ Regular eye pressure checks are essential during long-term steroid use.
✅ Early diagnosis and timely treatment can preserve vision.

Some memories don't fade with time—they become even more precious.

Even though our Sri Lanka trip was back in 2025 it still feels like yesterday. Every now and then, I find myself scrolling through those photos, reliving the laughter, the breathtaking landscapes, and the unforgettable moments.

Looking back, I realize that a truly memorable journey isn't just about the places you visit—it's also about the people who make those places come alive.
