He is a young thirty year old, strong physically, has a heart of gold, a bunch of tattoos and works in mental health with those who have developmental disabilities. A likeable man, who smiles often, has a slight bit of shyness, adores his friends and is still single.

R is having major surgery in a couple of weeks so we've been preparing for this event steadfast as he will be in hospital for seven days, an entire week.

Today we met with his new family doctor and completed the paperwork together so now he is supported by a physician local to him. Over our last few sessions in my office we've made lists, done interviews, placed phone calls, sourced resources, all to ensure everything is in place for his success and peace of mind.

This greatly anticipated massive undertaking that my client is finally completing is "Gender Confirmation Surgery, Phalloplasty".

What is even more cool, is that R, will be the first bottom surgery to ever be completed at VGH in their new surgical program run by a plastic and reconstructive surgeon, and a urologist. R IS THE FIRST!!

This mans courage and grit is so evident, he's about to be rewarded for his valiant effort with a body that aligns to his spirit.

Walking alongside R. as a witness to his journey, is an honor and a privilege that I hold dear professionally and personally.

This article from my hometown newspaper discusses the urgency for consent conversations/education to begin at a very young age between families and their small children to ensure they are equipped with the skill sets for personal safety from abuse.

It's disheartening and challenging that many families refuse to talk about consent, s*x, and safety with their young.

The deeper the conversations you share with your children, the more empowered and powerful they become.

The fewer conversations you share with your children, the more mistakes they will make and they will experience less self confidence.

Dig deep past your discomfort and find the courage to arm your children with the tools and science necessary to be as safe as possible.

Sexual assaults will likely never be eliminated in our society but much can be done to reduce their number and ensure a climate of respect in schools, workplaces and anywhere people congregate. . . .

The other night my husband and I went out for a walk in our neighborhood to enjoy all the Christmas lights and the fresh air too.

When we arrived back home finally, a unknown car was parked in our driveway and a person was knocking at the door.

Image the surprise, it was a client, who is very sick and with not much time left in life.

They had come here to deliver a most private package, a large bag containing all the items they do not want their family to run across when they pass away.

I was the chosen recipient of the objects of eroticism, s*xy memorabilia and mischievous products that had been collected over a life time.

This person has been a client sporadically over the last decade, we've seen one another easily at least 30 different times in sessions, classes and other unique settings.

It certainly is mindful and responsible to sort through ones belongings before dying to ensure that your most private and personal memories are taken care of with your wishes and comfort in mind.

Writing the final chapter in your own book of life is empowering and mentally freeing.

We still have our own personal Farewells to take care of between us, and have plans to meet soon to do this.

A knock at the door in the middle of a clients session time is always a disturbance, fortunately it does not happen too often.

This knock was loud, assertive and fast, it was jarring.

I left my office and headed towards the knocking to see who was there.

Once the door opens, I am startled.

My clients wife stands on the front step with a scowl on her face, eyes squinting and a demeanor of hostility.

Originally I saw them both together for three sessions, then the wife backed out of their remaining three sessions, unwilling to address the issues presented. So the husband has been seeing me alone since for lively discussions and support.

"I know he's in there with you".

"You are right, Bob is inside. What can I do for you today, are you coming in?"

"This is an emotional affair, if he's sharing with you about s*x or intimacy then its an affair."

"No, you are mistaken. Talking therapy does not constitute infidelity."

"I told him that we do not need help from anyone, our relationship and life together is-just-fine."

Suddenly Bob is next to me at the door and he says "Sara, why did you follow me here?"

"Because I wanted proof of your affair, you're supposed to be at work and instead you are here.

Before I can sort out what next to say in this delicate situation, Sara backs away and walks to her car, jumps in and pulls away.

Bob and I returned to my office and his session time as he's committed to nurturing his own self-esteem while also becoming a better communicator.

Grow yourself first.

I woke up to this request for help this morning, which is just a typical example of a (s*xual/intimate) health concern that people face in isolation.

Hello dear fine woman,

I would be interested in the information for a dr who would help Me possibly deal with a chronic issue with my rosebud. A hemorrhoid.

I think it’s just one? Maybe the length is close to the width of an average female finger?

Sometimes I get a fissure that seems to come from that same spot. I’ve had it since I was pregnant with my boys so a few decades now.

It annoys me tons and makes me uncomfortable with new lovers.

ON-LINE DATING SUCKS

She's been single for nearly a year and a half, no kids, just her beloved dog. A 34, she's an accomplished professional who owns her own condo.

She lives in a small community, works a 50 hour week and also volunteers regularly.

Dating has been complicated.

Using on-line platforms to meet men has left her feeling defeated and worthless.

She is trying to meet a responsible man who loves himself and life too, he also must be thoughtful and not shy in conversation or bed.

Nearly every man she's connected with has done something absurd, early on in their courting.

Men that ask for nudes.
Men that ask for or demand s*x on the first date.
Men that ghost.
Men that are only available during unusual hours.
Men that don't want to be known.
Men that touch without consent.
Men that want to only date casually, like FWB's.
Men that can't host.
Men who are cheating.

On-line dating is nasty and a difficult place to go to hoping to find a solid match to meet up with.

This woman is vibrant, sensual, kind and open, all she wants is to find a man that is open to exploring her mind as much as he wants her body. And eventually maybe be a long term partner, but right now, just an esteemed man who believes in courting and building trust so they can take this relationship to the next level of intimacy.

Often times it's helpful to get clear on what your deal breakers are so you don't up wasting time chasing the wrong person.

During our time together she's come to realize more specifically the type of man she seeks and what exactly her boundaries are with these new men.

While some boundaries may be flexible in nature, our non-negotiable boundaries are absolutely essential to our sense of being honored and respected.

They've been together for eighteen years and are a monogamous couple with no children at home. Each works full time and they are in their early 50's.

Their health is not the best, they are both overweight and diabetes, a mobility issue and a thyroid problem are shared between them.

Her weight has dampened her s*xual self-esteem and she refuses to be seen naked by any medical professionals, or her spouse, which causes much stress in their union.

His weight has distressed him enough that he's been calorie counting and walking again to keep at bay any additional winter poundage.

Together their intimacy is at an all time low, they have not had any s*xual relations in over fourteen months. There has been two attempts at rekindling shared touch that both ended poorly with much grief and sadness.

They have been seeing me for awhile now and we've made some great progress on discussions around being "vulnerable" and "touch starved".

He is desperate to connect with her in bed in any fashion and she has frantically created barriers to preclude this from happening.

She's waiting to feel "desirable".

He see's her as beautiful, s*xy and womanly now and always.

She sees herself as "fat, unattractive and not worth the effort". (her words)

Neither one wants to see the end of their s*xual intimacy, yet she's working through self-worth issues that make her unable to accept love in the form of pleasure, and he's lost his esteemed masculinity without the intimate contact.
