The care plan written on day one is almost never the same plan on day ninety.

Not because anyone did it wrong. Because people change. A shoulder gets stiffer. A doctor adjusts a prescription. Someone stops using the stairs, and nobody's quite sure when that started.

That's what our Quality Care Visits are for. Once a month, a senior member of our team sits down with the client and the caregiver, walks through the care plan, and makes sure it still fits the day. In Ottawa, that's Jasmine Hunt, RPN.

It's free for every client. And a written summary goes to the family afterward, posted with your regular caregiver reports.

Our caregivers are usually the first to spot when something shifts. That's where they act on what they've seen.

This week's post is different. It's written from the other side of the table.

Over 25 years we've heard older adults say the same things in living rooms across the country — rarely all at once, and almost never to their own children. So we gathered them into one letter.

There's a line in it that stops most families cold: it isn't the driving she cares about. It's getting to church on Sundays without having to phone anyone.

Nine words, and the conversation is over before it started. Your parent hears a verdict, handed down by someone who used to be their child.

Here's what works better: "I saw a new scrape on the bumper. What happened there?"

Same concern. Completely different door.
