Welcome to Healthcare Continuity Passport™

We often speak fondly about the soundtrack of our lives. A song can instantly transport us back to where we were, how we felt, the people we were with, and the emotions of a moment long past.

Our healthcare story is much the same.

From joyful moments such as birth, through illness, recovery, and the care we receive throughout our lives, our life story and our healthcare story are inextricably linked.

We travel. We move. We change doctors. We see specialists. Sometimes we find ourselves far from home and unexpectedly needing care. In those moments, physicians, pharmacists, nurses, and other healthcare professionals rely on information to help them make the best decisions possible.

Healthcare organizations around the world have invested heavily in managing health information. Hospitals, clinics, pharmacies, laboratories, and insurers retain and exchange information under applicable laws to deliver care.

But life does not always happen under ideal circumstances.

What happens when information is needed urgently, when loved ones are suddenly called upon to advocate, coordinate care, and make informed decisions on our behalf, or when the practical realities of accessing information across multiple organizations and geographies become challenging?

Empowering individuals to participate in the sharing of their own health information supports continuity of care while respecting privacy and consent requirements.

Throughout my career in healthcare, insurance, pharmacy, and digital health, I have seen how often people are asked to recall and repeat their health story—past surgeries, medications, allergies, diagnoses, and the names of physicians and specialists—often in moments of illness, stress, urgency, or lack of clarity due to aging, fear, or even loss of consciousness. In those moments, the burden of coordination and recall frequently falls upon individuals and the loved ones who care for them.

Perhaps the challenge is not a lack of information, but one of coordinating dispersed information and fragmented recall.

Globally, healthcare systems and standards bodies continue to advance the secure exchange of information to support continuity of care through efforts such as the International Patient Summary (IPS), a recognized framework for making essential health information available across providers, settings, and borders.

The Healthcare Continuity Passport™ was developed with these principles in mind. It reflects the recognition that while healthcare may be delivered by many organizations, individuals remain the constant across providers, systems, and borders. Our health story follows us throughout our lives.

Patient-Controlled Continuity of Care™ reflects the important role individuals and families can play in supporting continuity throughout their health journey.

Individuals are encouraged to complete and periodically review their Healthcare Continuity Passport™ with their family physician and other healthcare professionals. Clinician Verified™ reflects this collaborative approach, recognizing that continuity is strengthened when individuals and healthcare professionals work together.
