Lay out your last year of IPAC audits and look only at the dates.

Five in one week in March. Nothing until November. Four more in six days. All on weekdays. Three found something, and none have a follow-up note.

The individual audits may be excellent. But the pattern still shows a program running in bursts, and that is one of the first things anyone reviewing the file will notice.
Not a diligence problem. A scheduling problem, and a much easier one to fix.

Five possible reasons a practice is not happening. Only one is a training problem.
Start here: if everyone had perfect knowledge, would this still have happened? If the answer is yes, training is not the issue.
Empty dispenser? System gap. Audit results nobody saw? Feedback gap. Nobody owns it between audits? Accountability gap.
Run your last three findings through it.

“It’s single-use, but we sterilize it between patients, so it’s fine.”
It isn’t. Single-use devices do not have a validated reprocessing process for reuse. If an item cannot be thoroughly cleaned, sterilization cannot make it safe for reuse.
Reusable instruments come with validated reprocessing instructions. Single-use devices are intended for one use only.
Use once. Discard after use.

On larger healthcare construction and renovation projects, the anteroom at the entrance is not decoration. It functions as an airlock, helping maintain containment so construction dust does not escape into occupied areas.
Exact containment controls may vary by setting, project risk and available infrastructure, but the goal is the same: maintain a sealed barrier, controlled airflow and a safe transition between the construction zone and occupied space.
That may include sealed walls, self-closing doors, walk-off mats, HEPA filtration and pressure monitoring where required.
A poly sheet taped over a doorway is not an anteroom. Use the checklist during your site walk and verify all eight.

Eleven deadlines run an IPAC program. Some are daily, some are quarterly, and one is annual and easy to forget entirely.
The daily requirements are harder to miss because care is happening in front of you. The annual ones are the ones that slip, because nothing on the floor tells you a year has gone by.
Put all eleven in a shared calendar with a named owner beside each one. That single afternoon of setup can prevent a surprising number of missed obligations.

Sterilize, high-level disinfect, or just clean? For any dental instrument it comes down to what it touches. Enters tissue: sterilize. Touches mucous membranes: sterilize if you can. Touches only skin or surfaces: clean and disinfect. And when the manufacturer and the risk class disagree, always go higher. Most errors are one item sorted wrong once, then repeated. Pick three instruments. Can your team name the class?

Biosecurity has three jobs: keeping pathogens out, stopping them spreading inside, and stopping them leaving the building. Most clinics have a plan for the first two and nothing written for the third.
It shows up in specific places. Scrubs worn home and washed with family laundry after a ringworm case. Footwear going from the treatment area straight to the car. The carrier handed back to a client after an infectious patient. The technician who does a farm call in the morning and surgical prep after lunch.
None of that is carelessness. The procedures were all written facing the front door.

Fourteen findings from the last audit. All with the same thirty-day due date.
The ones that get closed first are usually the easiest ones, not the riskiest ones. That is how a missing signature gets fixed in a week and a shared piece of equipment keeps moving between residents for another quarter.
Two questions sort the list: how often does this happen, and what does it allow if an organism is already here.
Try it on your own list this week.

Before your IPAC Lead takes a week off, the most useful thing you can write down is not where everything is kept.
It is three short lists: what the backup decides on their own, what they phone about, and what waits.
"Call me if anything happens" sounds helpful. It leaves the hardest part, “deciding what counts”, with the person who has the least practice at it.

"It's outside, so it doesn't affect us."
The wall is not the boundary. The air handling system is.
Excavation, roofing, demolition next door and grounds work all throw fine dust, and it gets in through outdoor intakes, windows, and the door the crew props open for deliveries. Disturbed soil is a known source of Aspergillus.
Three questions decide it, and distance is not one of them. Where are the air intakes? Which door is the crew using? Which way is the wind running?
