Medication Safety is a Right, Not an Option.

Today, we are highlighting a fundamental principle of nursing practice:

The 7 "Rights" of Medication Administration.

Using this checklist before every dose is the single most effective way to prevent medication errors and protect the clients in your care.

The 7 "Rights" Checklist:

โœ… Right Medication: Verify the order and the label.

โœ… Right Client:* (See important note below!)

โœ… Right Dose: Double-check your calculations.

โœ… Right Time: Confirm the frequency and last given dose.

โœ… Right Route: Ensure the medication is given as intended (oral, IV, etc.).

โœ… Right Reason: Do you know why the client is receiving this drug?

โœ… Right Documentation: Chart immediately after administration.

๐Ÿ›‘ CRITICAL REFRESHER: Client Identification

The most crucial step in the "Right Client" check is properly identifying who you are treating.

Canadian practice standards are clear:

Use at least โ€œtwo client identifiersโ€ prior to administering medications.

Allowed Identifiers: Full Name, Date of Birth (DOB), or Health Authority ID Number.

โš ๏ธ NEVER USE: The client's room number. Room numbers can change, but a client's identity does not.

Letโ€™s break down the DNACPR (MOST) designations!

Transitioning to Canadian nursing means learning a whole new language of clinical documentation. If youโ€™re working in a VCH facility , youโ€™ll see these "M" and "C" levels on every chart.

Here is your quick-start guide to what they actually mean for your patient:

๐Ÿ”ด The "M" Levels (Medical Management)
These levels focus on supportive and therapeutic care without critical care interventions.

โ€ข M1: Purely comfort care. We manage symptoms (pain, breathing) and allow a natural death.

โ€ข M2: We treat acute conditions (like antibiotics for a UTI) but within the current setting. We donโ€™t transfer to acute care if the patient is in long-term care.

โ€ข M3: Full medical treatment and hospital admission, but no Critical Care (ICU).

๐Ÿ”ต The "C" Levels (Critical Care)

Think of "C" for "Critical Care." These patients are eligible for the ICU, but with one major distinction:

โ€ข C1: Everything possible except intubation/ventilation. We might use BiPAP or pressors, but we don't put them on a ventilator.

โ€ข C2: Full "Max Code" therapyโ€”including ICU, intubation, and mechanical ventilation.

โš ๏ธ Remember:

ALL of these levels are DNACPR. If the heart stops, we do NOT start chest compressions. We provide "everything up until the heart stops" based on the specific level.
