ND: Okay. Let's start from the beginning. *45-minute intake later* ND: Actually... you might be right about the iron. Not the other thing. But the iron. The moment when the clinical assessment lines up with the patient's intuition — just via a slightly more rigorous pathway — is one of the small joys of this work. Co-management is not just a documentation term. It is a clinical relationship — and when it works well, it produces better patient outcomes than either provider could achieve alone. What makes co-management work: → Both providers understand their respective roles from the start → Communication is structured, not ad hoc → The patient knows who is managing what → Documentation reflects the shared plan from both perspectives → Follow-up is built into the process — not left to chance The breakdown usually happens not in the clinical reasoning but in the handoff. Unclear referral letters. No follow-up communication. The receiving provider not knowing what the ND is continuing to manage. When a patients MD starts referring their other patients to you because of the clinical outcomes you produced together. *does not celebrate out loud* *celebrates enormously on the inside* Collaborative care working exactly as it should. Organic referral growth from good clinical work within a proper collaborative framework — it's one of the most consistent things ONIN members report. Not because ONIN markets them. The clinical work is only half of delegation. The documentation is the other half — and it is the part that protects you.When CoNO reviews a delegated act, they are looking at:
→ Evidence that a proper delegation agreement exists
→ That you have the training the delegating provider required
→ That informed consent was obtained and documented
→ That your clinical notes reflect the delegated act, rationale, and outcome
→ That communication back to the delegating provider is on file.
None of this is onerous when you have the right systems in place. ONIN members have access to SOPs, consent templates, and referral communication frameworks designed specifically for delegated IV therapies and collaborative care.Documentation is not a burden. It is what lets you keep doing the work.

A referral letter that actually gets read. A follow-up that closes the loop. A co-management note that tells the receiving provider exactly what they need to know. Good referral communication is a clinical skill. It is also one of the things that protects you professionally.
