North Simcoe Muskoka Self Management Programs Welcome to the North Simcoe Muskoka Self Management Programs! Our programs are designed to support adults living with one or more chronic health conditions.

Our programs include the Living a Healthy Life Series for individuals with chronic conditions or chronic pain, as well as skills-based workshops for health care providers to excel in their work with individuals with chronic conditions. The programs help people and their caregivers manage their symptoms, improve their quality of life, and reduce healthcare costs. Living with a chronic health condition can be challenging and we use evidence-based programs to teach the skills and knowledge you need to live a healthy life. Our programs include the Living a Healthy Life Series for individuals with chronic conditions, as well as skills-based workshops for health care providers to excel in their work with individuals with chronic conditions. Our programs are funded by the Ontario Ministry of Health & Long-Term Care and are free for anyone in Barrie, Orillia, Midland, Penetanguishene, Wasaga Beach, Collingwood, Muskoka, and all surrounding communities.
