Understanding : More Than Just Surface Peeling – The Root Cause is a Dual Defect in Skin Structure and Function
Many ichthyosis patients rely on moisturizing creams for a long time, which only temporarily softens the surface scales, never truly escaping the dryness and peeling. The core reason is that ichthyosis is not simply "dehydrated skin," but rather a combination of congenital skin structural abnormalities and impaired skin function, preventing surface moisturizing products from reaching the deeper issues.
The epidermis of a healthy person consists of four layers from the inside out: the basal layer, the spinous layer, the granular layer, and the stratum corneum. These four layers have distinct functions, with cells growing, maturing, and shedding naturally in an orderly manner, forming a complete and robust skin barrier.
However, patients with ichthyosis have congenital defects in epidermal development. The complete four-layer structure is destroyed, leaving only two functional layers. The crucial granular layer atrophies or even disappears, disrupting the normal cell differentiation process. New keratinocytes cannot mature properly, and old keratinocytes cannot be shed regularly, accumulating on the skin surface and eventually forming layers of raised, fish-scale-like scales.
Structural defects further induce severe skin dysfunction, most typically manifested as narrowing and blockage of the three major skin glands.
The sebaceous glands, sweat glands, and hair follicle glands are the three core tissues maintaining healthy skin: sebaceous glands secrete oil to lock in skin moisture; sweat glands are responsible for perspiration and regulating body temperature; and hair follicles ensure normal hair metabolism.
In individuals with ichthyosis, the abnormally thickened and accumulated keratin continuously compresses and blocks the gland openings, narrowing the channels and hindering passage. Sebum cannot be transported to the skin surface, resulting in a long-term lack of a natural oil protective film, rapid moisture loss, and persistent dryness and tightness; blocked sweat gland channels prevent the normal release of heat and sweat, leading to unbearable heat, stuffiness, and itchy skin; and long-term blockage of hair follicles by keratin plugs forms rough, goosebump-like bumps.
This also explains why ordinary moisturizing products have limited effects. Moisturizing creams and lotions only act on the outermost layer of the skin, providing temporary hydration and softening dandruff, but cannot repair damaged epidermal structures, let alone unclog the three major glands. Once you stop applying creams, dead skin cells continue to accumulate, and scales quickly reappear, creating a cycle of repeated treatments and relapses.
To fundamentally improve ichthyosis, you can't just focus on removing surface scales. You need to address two main aspects: firstly, gradually metabolize the excessively accumulated dead skin cells; secondly, unclog narrowed or blocked glandular channels to help the epidermal structure gradually repair itself and awaken the skin's own secretory, metabolic, and moisture-retaining abilities. Only when these three glands function smoothly again and the skin rebuilds its basic barrier can you break free from the long-term dependence on topical moisturizing products and achieve long-term, stable improvement.
