When a “Simple UTI” Isn’t Simple: Why We Built a Deeper Diagnostic Pathway for Young Patients

Body:

A 19‑year‑old woman came to us with what looked like a common problem: flank pain, dysuria, low‑grade fever. Her routine urine test was almost normal. She had already received multiple short courses of antibiotics elsewhere.

Instead of repeating the same approach, our team followed a structured pathway:

Clinical re‑assessment – revisiting the history and risk factors instead of assuming “recurrent UTI”.
Targeted investigations –
Ultrasound showed left‑sided pyelonephritis, mild hydroureteronephrosis, and crucially,
A retroperitoneal soft‑tissue lesion compressing the left ureter.
Integrated interpretation – anemia (Hb 9.2 g/dl) and elevated hs‑CRP supported an inflammatory or infiltrative process beyond a routine infection.
Escalation of care – the patient was referred for advanced imaging and multidisciplinary evaluation (urology, radiology, oncology) to rule out entities such as retroperitoneal fibrosis or lymphoma.
This case is a reminder that:

Over‑simplifying recurring symptoms can delay the diagnosis of serious disease.

At our centre, we are working to make “doing the right thing” the default:

Standardised evaluation protocols for recurrent UTI and flank pain
Close radiology–clinician collaboration to catch subtle but critical findings
Clear communication with patients, translating complex reports into understandable language
Early MDT involvement when imaging and labs don’t match the “simple” diagnosis
