Peptides are short chains of amino acids—think of them as small “messenger” molecules. Your body naturally uses peptides to signal processes like appetite, metabolism, healing, inflammation, and growth.

How much of the hype is real? Some peptides are very real medicine:

GLP-1 medications for diabetes/weight management are peptide-based (strong evidence).
Some peptide hormones are essential in endocrinology (e.g., insulin).

But a lot of what’s trending online is overpromised: “fat-burning peptides”, “anti-ageing injections”, “heal everything” claims, and products sold without proper regulation. Many are not approved, not consistently dosed, and not studied well for long-term safety.

My rule: If it’s not a regulated product with solid clinical evidence, be cautious—especially with anything sold online. If you’re considering peptides for weight, hormones, or metabolic health, choose the medical route with proper assessment and monitoring.

“Brain fog” isn’t a medical diagnosis — but it’s one of the most common things I hear from patients. It’s a non-specific cognitive symptom that can come from many systems in the body. Today, let’s break it down hormonally.

Hormones that can play a key role include:

Thyroid hormones (hypothyroidism can affect focus, memory, and mental speed)
Insulin and blood sugar (spikes and crashes can feel like fog, fatigue, and poor concentration)
Oestrogen/progesterone changes in perimenopause/menopause (sleep disruption and mood shifts can worsen cognition)
Cortisol rhythm (tired in the morning, wired at night, fragmented sleep)

The goal isn’t to label yourself — it’s to identify the driver and fix it with the right plan and the right tests.

With untreated hypothyroidism, weight can increase due to a slower metabolic rate and fluid retention. But once thyroid levels are properly treated and stable, the thyroid is usually not the only reason weight loss stalls.

What I check in real life:
✅ Are your levels truly optimised (TSH + free T4) and are you taking medication correctly?
✅ Are there other blockers: insulin resistance, poor sleep, high stress, low muscle mass, perimenopause, low protein intake, or hidden calories from “healthy” snacks?
✅ Is it fluid, fat, or muscle? Body composition matters more than the scale.

Good news: with the right plan, people with thyroid disease can absolutely lose weight — but it needs a metabolic approach, not just calorie cutting.

“All these weight loss injections are cheating.” Let’s challenge that.

If willpower was the whole story, nobody would struggle. Appetite, cravings, and body weight are strongly influenced by biology: hunger hormones, insulin, stress, sleep, and how your brain responds to food.

Medications like GLP-1 treatments work by changing the signals that drive overeating:
✅ reduce constant hunger and food noise
✅ help you feel full sooner
✅ improve insulin resistance and blood sugar control
✅ make it easier to follow a plan consistently

They’re not magic, and they still require good habits. But for many people, they level the playing field—especially when insulin resistance or metabolic problems are involved.

If you’ve tried “everything” and keep regaining, it may be time for a medical approach, not more self-blame.

In medicine, we don’t usually say “cure” for type 2 diabetes. What we aim for is remission—when blood sugar returns to the non-diabetes range without diabetes medication for a sustained period.

Remission is possible for many people, especially when type 2 is caught early, and it often requires:
✅ meaningful weight loss (often 10–15% for many, but individual)
✅ building muscle through strength training
✅ improving sleep, stress, and daily movement
✅ a nutrition plan you can maintain
✅ sometimes medications to get you there safely

Even in remission, the tendency can return, so ongoing habits and monitoring matter.

If you want a clear, medical plan for improving glucose and working towards remission, I can guide you step by step.

Metformin is not kidney-toxic. The kidneys mainly matter because they clear metformin from the body. If kidney function is significantly reduced, metformin can build up—so doctors adjust the dose or stop it based on your eGFR.

Simple rule: metformin is usually safe with normal or mild kidney impairment, and it may need dose reduction or avoidance in more advanced kidney disease. It should also be paused temporarily during severe dehydration, vomiting/diarrhoea, serious infection, or before certain contrast scans—because those situations can stress the kidneys.

If you’re taking metformin, the right move is not fear—it’s regular kidney checks and sensible sick-day rules.

Fasting can help some people by reducing snacking, improving insulin sensitivity, and supporting weight loss. But it doesn’t “balance all hormones” automatically. Your hormones respond to sleep, stress, nutrition quality, muscle mass, cycle stage, thyroid function, and overall energy intake.

For some, aggressive fasting can backfire: poor sleep, cravings, low energy, cycle disruption, and overeating later.
The best approach is individual.

If fasting works for you, keep it gentle and consistent (often 12:12 or 14:10), and focus on what matters most: protein, fibre, strength training, and stable routines.

A single insulin number needs context: when the sample was taken, what you ate the day before, stress, sleep, and even lab variation. Insulin also naturally rises after meals—so a non-fasting test can look “high” even in healthy people.

What matters more than one value:
➡️ Fasting glucose + HbA1c (your average blood sugar)
➡️ Fasting insulin with HOMA-IR (a better estimate)
➡️ Lipids (triglycerides/HDL) and waist size
➡️ Sometimes an OGTT (2-hour glucose test) if results are unclear

High insulin can still be an early warning sign—but the next step is to assess the full picture and create a plan, not panic.

Insulin doesn’t “create fat.” It’s a hormone that helps your body move glucose from the blood into cells to use for energy and to keep blood sugar in a safe range.

Weight gain happens when more energy is stored than used. Insulin can make storage easier in the right conditions—especially when insulin levels are high most of the day (often with insulin resistance and frequent high-calorie snacking).

In diabetes treatment, starting insulin can lead to weight gain because glucose is no longer lost in the urine and appetite often improves—this is a metabolic correction, not fat appearing from nowhere.

The goal isn’t to “avoid insulin.” The goal is to improve insulin sensitivity with the right plan.

GLP-1 medications are designed to be injected subcutaneously (into the fat layer under the skin), not into muscle. Using a longer needle or “going deeper” does not make the medicine stronger or faster. It can actually increase discomfort and bruising.

What really matters:
✅ Inject into the correct areas: abdomen, thigh, or upper arm
✅ Use proper technique and rotate sites
✅ Inject at a 90° angle with the device you’re prescribed
✅ Keep storage and dosing consistent

If weight loss or glucose control isn’t moving, the answer is usually dose titration, timing, side effects limiting intake, food choices, sleep, stress, and strength training—not needle length.

If you’re unsure about your technique or results, let’s review it properly.
