🩸 HbH Disease — When 3 α-Globin Genes Are Deleted
HbH disease occurs when 3 of the 4 α-globin genes are deleted:
−− / −α → 3 gene deletions 🧬
This causes a severe reduction in α-globin production, leading to excess β-globin chains that form HbH (β₄).
🔬 Key Laboratory Findings: • Hb ↓
• MCV ↓↓ markedly
• MCH ↓
• RBC count → Normal / ↑
• Reticulocytes ↑
• Iron studies → Usually normal
🩸 Peripheral Smear: • Marked microcytosis
• Hypochromia
• Target cells
• Anisopoikilocytosis
• Polychromasia
🔬 Supravital stain:
HbH inclusions may produce the characteristic “golf-ball” appearance.
🧪 Hemoglobin Analysis:
HbH (β₄) → May be detected
HbA → Predominant
HbA₂ → ↓
HbF → Usually normal or slightly ↑
💡 ASCP Pearl:
Marked microcytosis + hemolytic anemia + HbH inclusions → Think HbH disease.
🧠 Remember:
1 deletion → Silent carrier
2 deletions → α-thalassemia trait
3 deletions → HbH disease
4 deletions → Hb Bart’s / Hydrops fetalis
