High-Grade CD20-Positive Lymphoid Neoplasm with Very High Ki-67: What Should Be in Your IHC Panel?
A monomorphic high-grade CD20-positive lymphoid proliferation with a very high Ki-67 index requires a differential-directed IHC panel.
An initial panel may include Cyclin D1 and CD5 for blastoid mantle cell lymphoma; TdT ± CD34 for B-lymphoblastic lymphoma; CD10, BCL6 and MYC for Burkitt/high-grade B-cell lymphoma; and MUM1, CD138 ± light chains/EBER for plasmablastic or plasma-cell differentiation.
Final interpretation must integrate morphology, clinical findings and the complete immunophenotype.
