﴿وَمَنْ أَحْيَاهَا فَكَأَنَّمَا أَحْيَا النَّاسَ جَمِيعاً﴾ [المائدة: 32]
A case of male Patient 33 years old presented with stab abdomen with sharp object at The left hypochondriac region , vital data of the patient was very dangerous as : Blood pressure: 80/40 , Pules : 155 , Respiratory Rate: 30 , Temperature: 38 , RBS : 110 , Patient is pale , dehydrated, presented with hematemesis and melena just before the time of surgery , all Shock signs was presented .
Labs show : hemoglobin : 7 g/dL just before the time of surgery
FAST & CT abdomen and pelvis show : Rim of IPFF cant see any organs damage or possible of injuries.
The accident occurred 20 hours before he arrived at our hospital. He was treated at another hospital where he received First Aid, underwent examinations and Investigations , then seen by surgeons there .
The decision was Laparoscopic Exploration and deal with the possible
Injuries .
Under complete Aseptic conditions , A difficult General anaesthesia was done ; 4 ports was inserted ; A Regular Explanation was done ; we found after opening the lesser sac ; a perforation at the posterior wall of the stomach ; Repair was done with handling of the bleeding.
The Patient was lucky to proceed all The Surgery with laparoscopy .
Patient received all his medication at the operating theatre and pos-operative ; Patient was hospitalized for 4 days then discharged.
Now patient is vitally stable, on fall diet , open bowel , hemoglobin : 10.7
And back to his own life without any complaint.

حاله اليوم هي حاله طوارئ من الدرجه الاولى ؛ شاب ثلاثيني قد اصيب بطعنه نافذه لجدار البطن مما ادى الى تهتك بالأعضاء الداخليه مع وجود نزيف بالبطن وذلك لاكثر من ٢٠ ساعه قبل الوصول الينا .
المريض كان يعاني من فقد للدم ونزيف داخلي وذلك مما اثر ع علاماته الحيويه وادخل المريض في صدمه نزيفيه حيث ان ضغط الدم للمريض كان ٨٠/٤٠ ؛ ونبضه : ١٥٥ وارتفاع بدرجه حرارته ٣٨ مع تسارع بمعدل التنفس ٣٠ ؛ كل هذه المعطيات مع وجود فقر للدم : هيموجلوبين ٧ ونزيف من الفم وعند التبرز قد صعب المهمه بل تكاد تكون مستحيله لولا رحمه الله عز وجل وتوفيقه .
فقد قُدم للمريض الاسعافات الاوليه وسرعه اخذ القرار المصيري بدخول المريض للعمليات لاجراء جراحه دقيقه تتمثل في استكشاف للبطن بالمنظار الجراحي ؛ وتتوالى رحمات الله بالمريض فقد تم السيطره ع النزيف ورتق الجرح التهتكي بالجدار الخلفي للمعده وذلك دون الحاجه للفتح الجراحي التقليدي ومن ثم تم نقل المريض من غرفه العمليات لاستكمال علاجه وقد مكث بالمستشفى ٤ ايام ؛ واخرج بعدها خروج تحسن .
ومن لطف الله علينا وع المريض فانه الان يعيش حياه طبيعيه بدون شكوى من العمليه وما قبلها نسال الله له ولنا السلامه والعافيه .
