🧠 Can the words we use about back pain actually contribute to fear and disability?

“Degeneration.” “Disc bulge.” “Narrowing.” “Damage.”

In his Digi4MSK webinar, Professor Peter O’Sullivan reflected on how diagnostic labels, imaging findings and messages about having a “vulnerable back” can shape the way people understand their pain.

When people are repeatedly told to protect their back, avoid painful movements or interpret scan findings as evidence of damage, the consequences can go beyond the consultation: fear, hypervigilance, avoidance of activity and greater reliance on passive treatments.

As Peter highlights, findings on scans do not necessarily predict a person’s pain experience, disability or future trajectory.

🎥 In this extract, he raises an important question for clinical practice: How do we communicate back pain when there is no clear pathology?

Communication, beliefs about pain and person-centred care will also be part of the wider conversation at the Digi4MSK Final Congress on 19 September, with national and international experts sharing evidence and practical approaches to musculoskeletal pain care.

For World Physiotherapy Day, we want to recognise a profession whose role goes far beyond treating an injury.

Physiotherapists assess people’s needs, develop individualised treatment plans, use therapeutic exercise and other interventions, support rehabilitation and help prevent future problems.

But there is another part of their work that is particularly important to Digi4MSK: education.

Helping someone understand their condition, addressing concerns about movement, supporting self-management and making informed decisions together can be just as important as what happens during treatment itself.

And none of this happens in isolation. Physiotherapists are also an essential part of multidisciplinary teams working to provide coordinated, person-centred care.

Today, we celebrate all the physiotherapists helping people move, understand their health and participate more confidently in their care.

🧠 5 questions. One day to discuss them with clinicians and experts from different fields.

When caring for people with musculoskeletal pain, knowing the evidence is only part of the challenge.

The difficult questions often come when we try to apply it:

1. How do we know when self-management is appropriate?
2. How can our communication support behaviour change?
3. What do we do when someone is afraid to move?
4. How do we make self-management work in everyday clinical practice?
5. And what does implementing a biopsychosocial approach actually look like?

These are some of the questions we’ll explore at the Digi4MSK Final Congress, alongside discussions on integrated chronic pain services, the patient journey after discharge and the relationship between biomedical advances and biopsychosocial care.

🧠 Is distress in people living with chronic pain necessarily a separate psychiatric comorbidity?

In one of our Digi4MSK webinars, Professor Tamar Pincus challenged this assumption and invited healthcare professionals to look more carefully at how we understand and measure depression in the context of chronic pain.

As she explains, commonly used depression questionnaires include questions about sleep, energy, appetite, decision-making and mood. But when someone lives with persistent pain, can we always tell whether these responses reflect depression — or the experience of pain itself?
