🔱 New ESC 2026 Heart Failure Guidelines. 🫀🔝
☝🏻👨‍⚕️ The changes we've been waiting for:
🌟Respects the 2nd universal definition of HF: symptoms/signs + objective evidence of structural and/or functional cardiac abnormality. And stages A-D, with emphasis on the pre-HF concept (stage B).
🌟Classification into only 2 phenotypes: HF with preserved LVEF (≥50%) and HF with reduced LVEF (3 months of treatment).
🌟Obesity + HF ➡️ semaglutide or tirzepatide [IIa B], in HF LVEF >45% and BMI >30 kg/m² (only for ⤵️weight, ⤴️exercise capacity and quality of life).
🌟Exercise and cardiac rehabilitation: to improve functional capacity and quality of life, reduces 📉🏥 [class I].

Liver function tests

The liver does three jobs, and the panel splits cleanly into three groups:

Q1 - Are liver cells dying?
Enzymes reside inside hepatocytes and spill out when cells rupture.
High = injury, not failure.

1. ALT : the liver-specific leak.
Rises = hepatocyte damage.
Most specific to liver. (first marker of hepatocellular injury)

2. AST : the same idea, less loyal. Also in heart, muscle, RBC. (pair with ALT - see the ratio below)

AST:ALT ratio (De Ritis) :

Fatty liver, or hepatic steatosis, happens when excess fat builds up inside liver cells. It has become one of the most common liver problems worldwide, driven by obesity, sedentary lifestyles and diets high in refined carbs and sugar.

The condition often develops slowly over years. Many patients find out by chance during routine blood tests or an abdominal scan done for another reason.

Routine liver function test is required for early diagnosis.
