🫁 Top 10 Common Mistakes in Pulmonology Prescriptions
Mistake Why It’s a Problem Better Practice / Clinical Pearl:

1. Over-reliance on oral/systemic steroids for stable asthma or COPD Causes osteoporosis, diabetes, cataract, and infection risk.

Reserve systemic steroids for acute exacerbations only.

Use inhaled corticosteroid (ICS) maintenance when indicated.

2. Incorrect inhaler technique or failure to teach it Up to 70 % of patients misuse inhalers → poor control.

Demonstrate device use, check technique each visit, and match device type to patient ability.
3. Not prescribing a rescue SABA inhaler in asthmatics on maintenance therapy Patients lack emergency relief during attacks.
Always pair controller (ICS/LABA) with a reliever (SABA or SMART regimen if eligible).
4. Using LABA monotherapy in asthma Increases asthma-related death risk.
LABA must always be combined with ICS in asthma (e.g., formoterol + budesonide).
5. Unnecessary or prolonged antibiotics for viral bronchitis or COPD without sputum change Promotes resistance and C. difficile infection.
Use antibiotics only if purulent sputum + dyspnea + ↑ volume in COPD or confirmed bacterial pneumonia.
6. Failure to adjust drug doses in hypoxemic or CO₂-retaining COPD patients (esp. sedatives & opioids) Sedatives may depress respiratory drive → CO₂ narcosis.

❌. Avoid benzodiazepines/opioids unless palliative; if needed, use lowest dose under monitoring.
7. Prescribing cough suppressants (codeine, dextromethorphan) for productive cough Impairs mucus clearance → infection persistence.
Treat cause (infection, asthma, reflux); use expectorants or airway clearance instead.
8. Inhaled steroids in active pulmonary infection (esp. TB, fungal) May worsen infection or delay diagnosis.
Rule out active infection before starting ICS; monitor for hemoptysis/night sweats.
9. Oxygen overuse in COPD without target saturation Excess O₂ → CO₂ retention and acidosis......
