Two problems, one custom brace.

Foot drop: the front of the foot can't lift properly, so toes catch and trips happen. The Richie Brace Dynamic Assist has spring-loaded ankle hinges that help lift the foot by up to 15 degrees as you step through.

Severe adult acquired flatfoot (tibialis posterior dysfunction): the arch progressively collapses and the ankle rolls in. A custom ankle-foot orthosis, combined with targeted strengthening, is a well-supported first-line non-surgical option (Alvarez et al., Foot & Ankle International, 2006).

Meet Bronagh 👇

Bronagh Martin, BSc (Hons), HCPC-registered podiatrist at our Lisburn Road clinic in Belfast. Over six years with Lower Limb now, and a hugely valued member of the team.

🦶 Much of her week is general podiatry: corns, callus, thickened or fungal nails, cracked heels and verrucae. The everyday problems that make walking uncomfortable and are quick to fix in the right hands.

🩹 Her particular expertise is nail surgery for ingrown toenails, wound care and the diabetic foot, where regular assessment and careful routine care matter most. Patients with diabetes, poor circulation or reduced sensation see her for foot checks and ongoing care.

Sports injuries, MSK, and every Dublin clinic. Meet Darren 👇

Darren Costello is our sports and MSK podiatrist at Ormeau Road, Belfast, and he runs our monthly specialist clinics at Lower Limb Dublin in Clontarf.

🏃 His caseload is injury-led: runners, field-sport athletes and active people with heel, arch, Achilles, shin, ankle and knee pain that traces back to how the foot and leg load.

🔬 MSc Sports and Exercise Medicine (Ulster), CASE-accredited PGCert in musculoskeletal ultrasound (Brunel), Member of the Royal College of Physicians and Surgeons of Glasgow. In practice: he scans in clinic as part of the assessment, so diagnosis and plan happen in the same appointment.

💉 Ultrasound-guided injection therapy, corticosteroid or hyaluronic acid, when the scan and assessment support it. Guided means the needle is placed with the scan on screen, not by feel.

This patient presented just over a year ago with some pain in her big toe joint. We did a corticosteroid injection last year, with great results. The patient was able to walk pain-free for the last few years. However, recently she's been attending some concerts, standing for long periods at times, and not really supportive of footwear, and her symptoms had started to return.

She presented today to find an opinion on another injection, which we decided we should do, given that she responded so well from her previous injection. On the ultrasound scan, there was some inflammation present on Doppler, but the joint itself didn't look too bad. Range of motion was good, but it was still painful to put through her range of motion.

We opted again for another steroid injection and advised the patient that it will be a little bit tender for a couple of days, but it shouldn't impact her daily activities. Hopefully, we won't have any further flare-ups and maybe get another full year out of it. We have also advised on more supportive footwear, especially if she's going to be standing for long periods of time, and possibly some orthotics further down the line. At the moment, the patient is happy, and we will follow up with her again next week.

New school shoes bought in a hurry are how September starts for a lot of the children we see. Ten minutes in the shop fixes most of it:

1. Measure both feet, standing, in school socks. Fit the bigger one.
2. A thumb's width at the toe. Room to grow, not room to slide.
3. Laces, straps or a buckle. Slip-ons make the toes grip all day.
4. Firm heel, and a sole that bends at the ball of the foot, not the middle.
5. PE trainers and socks get the same rules. Recheck every term, growth comes in bursts.

The one habit that prevents ingrown toenails: cut nails straight across, not too short, and never dig down the sides. Tight shoes, sport and a growth spurt do the rest. If the edge of a big toenail goes red, swollen or sore, come early. A first episode is usually settled in one visit, and if it keeps coming back we fix it properly under local anaesthetic, children and teenagers included, parents in the room.

Diagnostic ultrasound Belfast: why we scan before we treat.

Ultrasound is a diagnostic tool. We use it alongside functional clinical tests to work out what is actually driving your pain, so treatment targets the right structure from the first session and you get on the quickest path to a good outcome.

It does not answer everything. Some problems still need MRI or X-ray, and when yours does we will say so and organise the referral.

The team keeps studying this area at postgraduate level. Darren has completed CASE-accredited MSK ultrasound training, Paul is enrolled now, and Chloe and Rory are next. That investment is about one thing: the best possible outcome for every patient.
