Preservation Rhinoplasty | Before & After

This patient presented with concerns regarding a dorsal hump, a broad nasal tip, and asymmetry when viewed from the front. Her preference was for a smoother bridge with either a gentle curve or a flatter dorsal profile.

On examination, the nose demonstrated a convex profile, reduced tip projection in relation to nasal length, no defined supratip break, and an acute columella–labial angle.

In rhinoplasty, these features cannot be considered independently. The bridge, nasal tip, projection, rotation, and frontal symmetry all contribute to the overall appearance and balance of the nose.

Following assessment, preservation rhinoplasty was discussed as a potential surgical approach. This technique aims to modify the shape of the nose while retaining as much of the existing structural framework as appropriate, rather than routinely removing and reconstructing the nasal bridge.

Careful pre-operative planning is essential. This includes assessing the internal and external nasal anatomy, discussing the patient’s preferred outcome, and determining whether further investigations, such as a CT scan, may be required before surgery.

Every rhinoplasty is planned individually according to the patient’s anatomy, concerns, and treatment objectives.

Rhinoplasty may be considered by someone who is unhappy with the appearance, proportion or structure of their nose, or who has difficulty breathing through it, and would like to explore whether surgery could address those concerns.

One of the most common reasons patients seek rhinoplasty is a dorsal hump, where excess bone or cartilage creates prominence along the bridge of the nose. Others may be primarily concerned with a broad or bulbous nasal tip, in which case a tip rhinoplasty may sometimes be appropriate.
Some patients present with a crooked or deviated nose caused by their natural anatomy or previous trauma. These cases may require a more structural approach to improve the shape and support of the nose.

Breathing difficulties are also a common reason for referral. Nasal obstruction may be related to a deviated septum, weakness or collapse of the internal or external nasal valve, enlarged turbinates, or changes following previous surgery or injury. Where the airway is affected, functional assessment forms part of the consultation, and any surgical plan may need to address both the structure and the appearance of the nose. Some patients have functional concerns alone; others have a combination of the two.

Rhinoplasty may also be considered following previous nasal surgery. Revision rhinoplasty is often more technically complex because the anatomy may have been altered and scar tissue may be present. Careful assessment and realistic expectations are therefore particularly important.

The most suitable procedure depends on the individual patient. The bridge, tip, septum, nasal support, facial proportions and breathing function all need to be assessed together before a surgical plan can be recommended.
There is no obligation to proceed with surgery, and no result can be guaranteed. A consultation provides the opportunity to discuss your concerns, understand the available options and decide whether rhinoplasty may be appropriate for you.

Why do I use a Keller Funnel during breast augmentation?

The Keller Funnel is a sterile implant delivery device that allows the implant to be inserted using a “no-touch” technique.
