Tesco fined £300,000 plus £34,279 costs for an accident that could have been avoided had the employee been fully trained to undertake the task he was carrying out.

There were other procedural and organisational failings relating to the task itself and the response to the accident.

Never underestimate the importance of good management systems, training and supervision when it comes to preventing and dealing with accidents.

Fines can't be insured against and the repetitional damage will be considerable, so overall it's a costly accident for Tesco, not to mention the employee who, at the time of prosecution, hadn't returned to work two and a half years after the accident.

This is a sad case from close to home. Please learn from it whether you use our services or not - look for new hazards and do something about them.

Despite what the spokesman says, the root cause isn't the hole; it's the organisational factors that led to the hole not being filled.
