Hair loss and thinning are among the most distressing things that can happen to your body — and among the most consistently dismissed. Told it is stress. Told it is age. Told it is genetic and nothing can be done. In most cases none of that is the whole story.

Hair follicles are among the most nutritionally demanding cells in the body. They require a continuous and abundant supply of iron, oxygen, hormones, vitamins and minerals to remain in the growth phase. When the body becomes deficient in any of these — or when the hormonal environment shifts — follicles are among the first to be sacrificed. This is not a cosmetic problem. It is a biological signal.

The most common cause of hair loss in women is low ferritin. Not anaemia — ferritin. Ferritin below 40 starves follicles of the oxygen they need to sustain the growth phase, and shedding begins months before any fatigue or other symptoms appear. Most standard blood tests do not check it. Most women with significant hair loss have never had it measured.

The second most common cause is thyroid dysfunction. Both underactive and overactive thyroid cause hair loss — and both are missed when only TSH is tested. T3, T4 and thyroid antibodies are all necessary for an accurate picture.

Hormonal imbalance — particularly the DHT elevation and SHBG reduction pattern — drives the crown thinning and hairline recession that becomes more common from the mid-30s onwards. Vitamin D deficiency impairs the hair growth cycle directly. Low zinc disrupts follicle protein synthesis. B12 and folate deficiency accelerates shedding by disrupting cell division in the follicle itself.

This is not a post about replacing therapy or medication. Both matter. Both work. This is about the layer of mental health that almost nobody is talking about — and that almost nobody is testing for.

Serotonin, dopamine and GABA — the three neurotransmitters most directly responsible for mood, motivation and anxiety — cannot be produced without specific nutrients. B6, iron, zinc, folate and magnesium are not optional extras for brain chemistry. They are the raw materials without which the brain simply cannot manufacture the chemicals that govern how you feel.

Low ferritin — even within the normal range on a standard test — directly reduces serotonin and dopamine production. This is not a theory. It is biochemistry. Low Vitamin D is one of the strongest independent predictors of clinical depression. Low B12 and folate impair methylation — the process by which neurotransmitters are synthesised and regulated. Low magnesium keeps the nervous system in chronic activation — producing an anxiety that no amount of mindfulness can fully resolve because the nervous system is not calming down for a biochemical reason, not a psychological one.

And underlying all of it — the thyroid. Hypothyroidism produces depression, anxiety and cognitive impairment that is clinically indistinguishable from psychiatric illness. This is one of the most common and least discussed experiences in women’s fitness — and it is almost never about what you are doing in the studio. It is about what is happening inside your body before you even get there.

Low ferritin — below 30 — directly impairs oxygen delivery to muscles. Your body cannot repair and build effectively after sessions regardless of how well you train. A standard GP blood test will not flag this until it is much lower.

Thyroid dysfunction slows metabolism, increases fat storage and impairs muscle protein synthesis — making meaningful body composition change genuinely difficult regardless of training volume. Chronically elevated cortisol actively breaks down muscle tissue and increases abdominal fat storage — directly opposing the goals of reformer pilates. And declining oestrogen and testosterone from the mid-30s onwards reduce the body’s ability to build and retain the lean muscle that pilates is specifically designed to create.

None of this shows up in a standard blood test. All of it is measurable with the right panel. And all of it is addressable once you know what you are dealing with.
