Your first aesthetics appointment should mostly be talking.

Not mostly injecting. Not a form on arrival and a treatment before you have spoken to anyone but a conversation.

At Moorland Aesthetics, new patients do not have treatment on the same day as their first appointment. The consultation is where the clinical picture is formed, where concerns are heard properly, where realistic outcomes are discussed, and where the decision gets made.

The form comes at the end, or even in the next appointment. After the conversation. After the decision.

If your previous experience of an aesthetics appointment started with a clipboard and ended with a needle, that is a formality dressed up as a consultation. It is not the same thing.

I often get asked when should I start having aesthetics treatments?

That is the wrong question. Here is the right one.

Do I have a specific concern that bothers me, that treatment could reasonably address, and that I have thought about carefully enough to make an informed decision?

The answer to that question does not depend on your age. Some people have a concern at 28 that is worth addressing. Some have the same concern at 48 and are entirely comfortable leaving it. Both are valid. The decision should be driven by what actually bothers you, not by age, not by a trend, not by what someone else is doing.

What I think matters before starting: your skincare should be in place first. Not because skincare replaces treatment, but because treatment on a protected, well-hydrated skin produces results that last better and look better. SPF. Retinoids. A consistent routine. The foundation matters.

And the timing should be for yourself, not for an event. Treatments booked for a specific occasion are almost always booked with insufficient time for proper consultation and reflection.

If you are asking the question that is probably a sign you are ready for a consultation. Not necessarily a treatment. The consultation.

There are things I don’t offer at Moorland Aesthetics. I think it tells you as much about the clinic as the list of things I do.

I don’t do before-and-after content. Not because I don’t get good results but because the format creates unrealistic expectations for the next patient. One result isn’t a promise.

I don’t use urgency tactics. No limited-time offers. No ‘only three slots left.’ If a treatment is right for you, it’ll still be right in three weeks when you’ve had time to think about it.

I don’t treat on the same day as a first consultation. I get pushback on this sometimes, people are here, they’re ready, but the reflection period exists for the patient’s benefit, not mine. Occasionally someone comes back and says they’ve changed their mind. That’s the point.

I don’t offer treatments I can’t defend clinically. If the evidence isn’t there, or the risk profile isn’t one I can discuss honestly, it doesn’t go on the list. There are things in aesthetics I’m watching with interest but won’t offer yet.

None of this makes Moorland Aesthetics better than every other clinic. It makes it the clinic I want to run.
Which turns out to be mostly the same thing.

Three things ageing your skin faster than you realise.

Not wearing SPF every day. UVA is there when you cannot see sunlight. It goes through glass. It is the biggest preventable driver of visible skin ageing.

Sleeping on your face. Compression from pillowcases contributes to permanent lines over time, especially around the eyes and mouth. A silk pillowcase can really help.

Repeated significant weight changes. Collagen does not fully recover from repeated cycles of stretching and contracting. Stable is better than the same average weight reached by yo-yoing.

None of these need a treatment. Start with the SPF.

A question I get asked regularly is why do treatments cost what they do. It’s a fair question and here is the straightforward answer.

The products. I use CE-marked products with documented clinical data. They are not the cheapest option available. I choose them because the evidence supports them and I want to know exactly what I am injecting. Product cost is a meaningful part of treatment pricing.

The qualifications. I am a GP, this means a five-year medical degree, foundation training, and GP specialty training. Over 10 years of clinical education before aesthetics training was added on top. That background shapes every clinical decision: medication history, drug interactions, anatomy, risk assessment, emergency management.

The safety infrastructure. I carry emergency medication including adrenaline. I maintain current BLS and anaphylaxis training. I have specialist indemnity insurance covering aesthetics. Clinical records are kept properly. Consent is a process, not a form on arrival.

The time. Appointments are long enough to do the job properly. That limits how many patients I see per session. It is a deliberate choice.

This is what the price reflects. Not a brand premium or a postcode. The clinical standard the treatments are delivered to.

Lip filler is not one thing.

It can mean a small amount of hydration and softening, almost imperceptible, but the lips feel and look different to you in a way that matters.
It can mean defining the vermilion border without changing the volume at all.
It can mean a little more projection without any additional width.
It can mean correcting an asymmetry that has always bothered you in photographs.
It can mean the fuller, more visible result that some people genuinely want and are entirely clear about wanting.

The version that is right depends on what you are actually trying to address, which is why the conversation matters more than the treatment.

What I find most interesting about lip work is the gap between what people ask for and what they actually want. Someone who comes in asking for fuller lips often, once we start talking, wants something much more specific; more definition, less thinning at the edges, a little more shape. The volume was a proxy for the real concern. Getting to the real concern changes what I would suggest.

The most successful lip results are often the least obvious. Not invisible, just entirely in keeping with the face they belong to. That is the target.

If you have been unsure about lip filler because you do not want a particular look then it is worth knowing that the particular look is one option among many. A consultation is where we work out which option, if any, is actually yours.

Three things people get wrong about botulinum toxin that it’s worth correcting before you book.

It is not permanent. The effect wears off completely, usually within three to four months. Muscles recover fully. If you stop having it, your face returns to where it was. Nothing that happens with properly administered botulinum toxin is irreversible.

Natural does not mean less. It means the right amount, placed accurately, by someone who has assessed your specific anatomy and movement. Under-injecting produces inconsistent results as often as it produces subtle ones.

The result is not immediate. The full effect takes up to fourteen days. Day two is not a fair assessment. If something looks off at day two, wait. If it still looks off at day fourteen, that is when to contact your practitioner. When starting it can take adjustments to get the dosing and placement right for your specific anatomy

The question I am getting most often about polynucleotides right now is not about how it works. It’ “what will I actually see? What will be different about my skin?”
 
Here is the honest answer.
 
Most people notice improved hydration around their second session. The skin holds moisture differently, feels more comfortable and less reactive. There isn’t a visible plumping effect but a change in how the skin functions.
 
Texture builds more slowly. The surface becomes more even, fine lines caused by poor skin quality (not those caused by muscle movement) begin to soften. Not because they have been filled but because the tissue underneath has improved.
 
The third thing is harder to describe but it is what patients say at their review. They look less tired, someone asked if they had been away, skin just feels better. That is tissue quality. It does not show dramatically in a photograph but feels amazing.
 
What PN will not do:
-Produce an immediate visible result after one session.
-Create volume or lift.
-Generate a dramatic before and after.
If that is the expectation, this is not the right treatment.
 
What it will do, over a full course and the time it needs is improve how your skin actually works.

The time. Appointments are long enough to do the job properly, which limits how many patients I see per session. That is a deliberate choice so that my clients have the time they need.

This is what the price reflects. Not a location premium, not a brand markup, the clinical standard the treatments are delivered to.

How long does filler last? The honest answer — and what you can actually do about it.

Twelve to eighteen months is the figure quoted most often. It is not wrong, but it is optimistic for many patients and many areas. Lips: typically six to nine months. Cheeks and midface: twelve to eighteen months is more realistic in lower-movement areas. Tear trough tends to persist longer. The variation between individuals and between areas is significant.

Why the variation? The treatment area matters — high-movement areas break down product faster. The product itself matters — softer, more fluid fillers metabolise more quickly than denser, more cross-linked formulations. And individual metabolism matters. Some people break down filler substantially faster than others. You find out over time.

What you can do to extend results:

SPF every day. UV exposure degrades the surrounding tissue quality, which directly affects longevity.

Avoid prolonged heat — saunas, hot yoga, very hot environments — particularly in the first two weeks, and as a longer-term habit.

Stay well hydrated. Hyaluronic acid is water-attracting. Well-hydrated tissue supports the result.

Retinoids consistently. Patients on a regular retinoid tend to hold results noticeably better because the underlying tissue is in better condition.

None of these are complicated. They are the difference between a result that lasts eight months and one that lasts fourteen.
