Back Pain in Pregnancy

1. Clinical Classification

Guidelines recommend first distinguishing:

A. Physiological musculoskeletal pain (most common)

Lumbopelvic pain

Postural/mechanical pain

Sacroiliac dysfunction

Symphysis p***s dysfunction

B. Secondary/pathological causes (must exclude)

Urinary tract infection / pyelonephritis

Preterm labor

Renal colic

Disc prolapse / neurological compression

Trauma

2. Red Flags (Urgent Assessment Required)

investigate urgently if:

Fever or urinary symptoms → possible UTI/pyelonephritis

Rhythmic abdominal tightening → possible preterm labor

Neurological signs (weakness, saddle anesthesia, bladder/bowel issues)

Severe unilateral flank pain → renal colic

History of trauma or fall

Pain that is severe, progressive, or not mechanical in nature

3. First-Line Management

A. Non-pharmacological (FIRST LINE)

Exercise and physiotherapy

Pelvic stabilization exercises

Core strengthening

Swimming or walking

Pelvic Tilt is commonly recommended in physiotherapy programs

Manual therapy

Physiotherapy

Osteopathic manipulation (trained providers only)

Supportive measures

Pregnancy pelvic/support belt

Posture correction

Avoid prolonged standing/sitting

Heat therapy

Warm compress (safe, not hot)

Sleep positioning

Side-lying with pillow between knees (recommended in guidelines)

4. Pharmacological Management (If Needed)

First choice:

Paracetamol (Acetaminophen) → considered safe in pregnancy (ACOG)

Avoid / caution:

NSAIDs (especially after 20 weeks → risk of fetal renal effects & oligohydramnios; contraindicated in 3rd trimester per FDA/ACOG)

Opioids only in severe cases and short duration under supervision

5. Special Conditions (Guideline Notes)

Pelvic Girdle Pain (PGP)

Common in 2nd–3rd trimester

Managed primarily with:

Physiotherapy

Pelvic belt

Activity modification

Avoid asymmetric loading (stairs, single-leg standing)

Symphysis P***s Dysfunction

Waddling gait, p***c pain

Managed similarly with stabilization and support

6. When Imaging is Needed

MRI is preferred if neurological deficit or suspected disc pathology

Avoid CT unless absolutely necessary

7. Prognosis (Guideline Consensus)

Most cases are self-limiting

Improves postpartum in majority of women

Recurrence in subsequent pregnancies is possible

Vaginismus is a condition in which the muscles of the pelvic floor surrounding the va**na involuntarily tighten or spasm whenever va**nal pe*******on is attempted or anticipated. This reflex contraction can make s*xual in*******se, tampon insertion, or even a pelvic examination difficult, painful, or impossible.

Symptoms

Difficulty or inability to achieve va**nal pe*******on.

Pain or burning during attempted in*******se (dyspareunia).

Fear or anxiety related to pe*******on.

Involuntary tightening of the pelvic floor muscles.

Normal s*xual desire and arousal may still be present.

Causes

Vaginismus is usually multifactorial and may involve:

Fear of pain or previous painful in*******se.

Anxiety, stress, or psychological trauma.

Sexual abuse or negative s*xual experiences.

Pelvic floor muscle dysfunction.

Underlying gynecological conditions (e.g., vulvodynia, recurrent infections, endometriosis, lichen sclerosus).

Sometimes no identifiable cause is found.

Diagnosis

Diagnosis is based on:

A detailed medical and s*xual history.

Gentle pelvic examination (only if tolerated).

Excluding other causes of ge***al pain.

Treatment

Treatment is highly effective and often includes:

Patient education about pelvic anatomy and the pain cycle.

Pelvic floor physical therapy with muscle relaxation techniques.

Graduated va**nal dilator therapy.

Cognitive Behavioral Therapy (CBT) or s*x therapy to address anxiety and fear.

Treatment of any underlying gynecological condition.

In selected refractory cases, botulinum toxin (Botox) injections into the pelvic floor muscles may be considered, although evidence is still evolving.

Prognosis

Most women experience significant improvement or complete resolution with a multidisciplinary approach involving gynecologists, pelvic floor physiotherapists, and psychological or s*xual health professionals. Early diagnosis and individualized treatment are associated with the best outcomes.
