MRCPI OSCE Structured Discussion Practice

Case brief for brainstorming🧐

Aisha Khan is 31 years old, G0, attending the renal obstetric clinic to discuss pregnancy planning

She has CKD stage 4 due to reflux nephropathy.

Relevant results:

eGFR 22 ml per minute per 1.73 m2
Creatinine 245 micromol per litre
BP 142 over 88 mmHg on labetalol
Proteinuria 850 mg per 24 hours
Hb 96 g per litre
No diabetes
Not on dialysis

Before looking at the answer, think like the examiner:

What is the effect of CKD on fertility?
How will you assess her before pregnancy?
What are the maternal and fetal risks?
How can pregnancy affect her renal disease?
What is your preconception and antenatal care plan?

This is a very important maternal medicine OSCE topic, and the key is not to give general reassurance. The candidate must recognise that CKD stage 4 is a very high risk pregnancy and needs proper renal obstetric counselling.

🤰Sarah Walsh is 29 years old, G1P0, currently 10 weeks pregnant.

She has cystic fibrosis diagnosed in childhood, with pancreatic insufficiency and chronic chest disease.

Relevant results:

FEV1 68 percent predicted, stable over the last year
BMI 20
Oxygen saturation 96 percent on air
Previous sputum grew Pseudomonas aeruginosa
No known Burkholderia cepacia
Impaired glucose tolerance
Partner carrier status not yet tested

Examiner structured discussion:

1. What is the effect of cystic fibrosis on fertility?
2. What are the maternal and fetal risks of pregnancy with cystic fibrosis?
3. What is the effect of pregnancy on disease progression?
4. What is your preconception plan?
5. What is your antenatal care plan?
6. How will you plan intrapartum care?
7. What is your postnatal care plan?

Think like the examiner 😎

What are the key risk factors?
What thresholds matter?
When is pregnancy high risk or contraindicated?
What must never be missed in counselling?

📚 MRCPI Part 2 — Topic of the Day

🔴 Ectopic Pregnancy — Initial Management

When assessing a woman with suspected ectopic pregnancy, the first priority is always to assess her clinical stability.

🔹 Unstable patient
→ Immediate resuscitation + urgent surgical management.

🔹 Stable patient
→ Management depends on:
• β-hCG level
• Ultrasound findings
• Ectopic pregnancy size
• Presence/absence of fetal cardiac activity
• Symptoms and clinical condition
• Patient’s ability to return for follow-up

⚠️ Exam Tip:
Never choose medical or expectant management before considering the woman’s clinical stability.

🩺 Think clinically. Manage the patient, not just the β-hCG number.
