Frozen Shoulder vs Rotator Cuff Tear: Know the Difference
Shoulder pain is common, but not all shoulder conditions are the same. Frozen shoulder (adhesive capsulitis) and rotator cuff pathology are often confused, yet they have distinct clinical presentations.
What is Frozen Shoulder?
A condition where the shoulder capsule becomes inflamed and thickened, causing progressive stiffness and pain. It typically develops in three stages: freezing (pain dominant), frozen (stiffness dominant), and thawing (gradual recovery). More common in ages 40-60, especially with diabetes or thyroid disorders.
What is Rotator Cuff Pathology?
Injuries to the four muscles and tendons that stabilize the shoulder—from tears and tendinitis to impingement. Can result from trauma, repetitive activities, or age-related degeneration.
Key Clinical Differences:
🔍 Range of Motion:
Frozen Shoulder: BOTH active AND passive motion severely restricted in all directions
Rotator Cuff: Active motion limited, passive motion preserved
🔍 Pain Pattern:
Frozen Shoulder: Deep aching pain, worse at night and end-range movements painful. Frozen Shoulder: Shows classic capsular pattern with external rotation most limited, followed by abduction, then internal rotation
Rotator Cuff: Pain with specific movements, painful arc between 60-120 degrees
🔍 Strength:
Frozen Shoulder: Appears weak due to stiffness, but muscle power intact
Rotator Cuff: True weakness in specific muscles (positive drop arm test, lag signs, lift off test , belly press test , neers test )
🔍 Onset:
Frozen Shoulder: Gradual, no specific injury, progressive over months
Rotator Cuff: Sudden (trauma) or gradual (degenerative)
Why This Matters:
Treatment differs significantly. Frozen shoulder needs mobility-focused therapy; rotator cuff injuries may require strengthening or surgery for complete tears.
When to Seek Help:
Persistent pain beyond 2-3 weeks, difficulty with daily activities, night pain, or progressive stiffness warrant professional evaluation.
