Elderly male with hematuria ↘️↘️↘️
Most bladder tumors in elderly patients are urothelial, or transitional cell, carcinomas. Definitive diagnosis requires cystoscopy with biopsy, while urine cytology and contrast-enhanced CT or MRI are essential for accurate staging and treatment planning.
Key ultrasound features suggestive of a bladder tumor include:
* An irregular or papillary intraluminal soft-tissue mass.
* Fixed attachment to the bladder wall.
* Internal vascularity on Color Doppler.
* Focal bladder wall thickening with loss of the normal wall interface.
* Associated enlargement of the prostate with features of bladder outlet obstruction or possible prostatic invasion.
Additional secondary findings that support long-standing urinary obstruction include:
* Increased post-void residual urine.
* Thickening and trabeculation of the urinary bladder wall.
* Bladder diverticula in chronic cases.
* Hydroureter and hydronephrosis when obstruction is severe.
These associated findings do not confirm malignancy by themselves. Instead, they indicate chronic bladder outlet obstruction, which may coexist with or result from an underlying bladder tumor.
