Beyond Pain: Rethinking Chronic Pain Through the Neuroimmune Orchestra

What if chronic pain isn't simply a problem of injured tissues or overactive nerves?

What if pain persists because communication within the body's neuroimmune network has become unbalanced?

In this keynote presentation, I explore a new way of understanding persistent pain—using the analogy of an orchestra—to explain how neurons, immune cells, glial cells and the gut–immune axis continuously communicate, and how restoring harmony may be just as important as treating the symptoms themselves.

This evidence-based lecture discusses:
✅ Neuroimmune communication in chronic pain
✅ Why inflammation is more than an "on-off" process
✅ Active resolution biology and specialized pro-resolving mediators
✅ The gut–immune axis and its role in pain
✅ Precision pain medicine and where the future may be heading

Whether you're a pain physician, anaesthetist, orthopaedic surgeon, neurologist, rheumatologist, physiotherapist, rehabilitation specialist, researcher or a healthcare professional interested in chronic pain, I hope this presentation offers a fresh perspective.

Ultrasound-Guided Upper Limb Pain & Regional Anesthesia Interventions Workshop 14–15 March 2026

The one skill that truly changes outcomes…
POCUS- Point Of Care Ultrasound.

With experience, you realize— clinical knowledge alone is not enough.

What matters is the ability to visualize anatomy in real time and act with precision.

Over two days (14–15 March 2026), this workshop focused on exactly that.

Not just techniques— but how and when to use them.

• Real-time ultrasound-guided scanning
• Stepwise approach to upper limb interventions
• Practical insights into complications and safety
• Case-based discussions and decision-making
• Hands-on supervised learning
• Cryoablation demonstration

Beyond the sessions, what stood out was the depth of discussion—questions shaped by experience,
and answers grounded in real practice.

Because ultimately, in interventional pain management - precision defines outcomes.

14–15 March 2026

Two days.
Intense learning.
Ultrasound machines running non-stop.

Our Ultrasound-Guided Upper Limb Pain & Regional Anaesthesia Interventions Workshop (14–15 March 2026) brought together clinicians who wanted to move beyond theory and truly understand how these procedures are performed in real clinical practice.

Over the two days we explored:
• Live ultrasound scanning and anatomy on volunteers
• Step-by-step procedural techniques
• Real clinical insights with interactive discussions on technique, complications, and clinical decision-making
• Problem-solving from real case scenarios

RADIAL NERVE - ULTRASOUND SCANNING | Spiral Groove → Deep Branch → Superficial Branch

Ultrasound-guided nerve scanning is a core skill for modern pain physicians and regional anaesthesia practitioners.

In this video, Dr. Amod Manocha demonstrates a systematic ultrasound approach to identifying and tracking the radial nerve, beginning at the spiral groove of the humerus and following it distally as it divides into the deep branch (including posterior interosseous nerve) and the superficial radial nerve.

In this live patient scanning demonstration, you will learn:
✔ How to locate the radial nerve in the spiral groove
✔ Step-by-step distal nerve tracking technique
✔ Identification of the radial nerve bifurcation
✔ Differentiating the deep branch vs superficial radial nerve
✔ Practical ultrasound landmarks for accurate identification

This step-by-step scanning technique is extremely useful for clinicians performing:
✔ Ultrasound-guided nerve blocks
✔ Pain interventions
✔ Entrapment neuropathy evaluation
✔ Regional anesthesia
✔ Musculoskeletal ultrasound

These scanning techniques are exactly the type of hands-on skills we teach in our upcoming workshop.

Rib Fracture Pain: Are We Matching the Healing Timeline?

🫁 Rib fracture pain is not just about pain relief — it’s about physiology, ventilation, and outcomes.

Most of us were trained to manage rib fracture pain with:
💊 Paracetamol, NSAIDs, opioids
🧠 Thoracic epidural
📍 Paravertebral block
🟦 ESP or serratus plane blocks
🪡 Intercostal nerve blocks
All of these work.

But here’s the uncomfortable question 👇
👉 Do they last long enough to match the biology of rib fracture pain?

In this video, Dr. Amod Manocha, Pain Specialist, takes a hard, evidence-based look at why rib fracture pain behaves differently from most injuries — and why duration of analgesia matters more than we’ve been taught.

🎬 WHAT YOU’LL LEARN IN THIS VIDEO
✅ Why rib fractures cannot truly “rest”
✅ How pain-limited ventilation leads to atelectasis, pneumonia, failed extubation, and prolonged ICU stay
✅ The real-world limitations of thoracic epidurals, paravertebral catheters, ESP blocks, serratus plane blocks, and intercostal nerve blocks
✅ Why short-acting regional techniques often fail to match the timeline of rib fracture pain
✅ Where intercostal nerve cryoablation fits — and why it can change outcomes in selected patients

🚨 This is not about abandoning conventional regional anesthesia.
It’s about asking a better question:
Does my analgesic strategy truly match the duration and physiology of rib fracture pain?

❄️ WHY INTERCOSTAL CRYOABLATION DESERVES A PLACE IN THE CONVERSATION
Compared to epidurals and catheters:
✔️ No sympathetic block
✔️ No hypotension
✔️ No indwelling catheters
✔️ No continuous local anesthetic infusion
✔️ Lower nursing burden

Compared to repeated intercostal blocks:
✔️ Prolonged analgesia lasting weeks
✔️ No repeated needle trauma
✔️ No cumulative local anesthetic toxicity
For elderly patients, multiple rib fractures, post-CPR injuries, and polytrauma, this approach can be outcome-changing — not just comfort-improving.

🎓 PART 2 — PRACTICAL, ANATOMY-DRIVEN (DON’T MISS THIS)
📌 In Part 2, Dr. Amod Manocha will break down:
🧠 Intercostal nerve anatomy
📍 Paraspinal vs paravertebral vs posterior, mid-axillary, and anterior approaches
🪡 Where blocks, cryoablation, and RF actually work best
⚠️ Why where you intervene often matters more than what you do
