Finally me and the patient get the desired results!
What’s I’ve learned from this difficult surgery of transconjunctival lower lid blepharoplasty is:
When patient with puffy lower lids come to you with a history of puffiness started for e.g 2 years ago, this is ok, BUUUT,
If you discovered, by chance, that your patient with puffy lids was obese and then he became thin, but his/her lids remained puffy, you are facing a difficult case of LLB, SOOO,
first, try to escape 😅, 2nd, if you can’t, at least for the sake of your brains cells, and for the sake of the patient, do it under GA !
In the first scenario, which is the usual scenario, the lower lid fat pads are pr*****ed from its normal anatomical location, which is … ok.
In the second scenario, the fat pads are in their NORMAL anatomical position, and you will go to that position, and you’ll, try to remove them!
