63 years old female presented with firm submandibular swelling related to submandibular gland Rt side,regarding clinical and radiological examination showing well defined tumor attached to deep structures, mostly looks benign from history,clinical examination and radiological features.
Total excision of tumor together with submandibular gland and preservation of marginal and Hypoglossal nerves.
Surgery done by Dr.Sabah Abdulaziz/consultant of oral and maxillofacial surgery.
تم ولله الحمد اجراء عملية رفع ورم حميد مع الغدة اللعابية تحت الفكية الجهة اليمنى لمريضة بعمر ٦٣ عام ،تمت العملية بنجاح وخرجت المريضة متحسنة ولله الحمد.

Old woman 64 years,presented with exophytic ulcer of LT side of alveolar mucuosa of lower jaw at molar region with mobility of associated teeth and erusion of underlying bone seen with CBCT , PET scan showing increased SUV of primary lesion with suspicion of lymph node metastasis at level IIA ,incisional biopsy reveals well differentiated squamous cell carcinoma,
Marginal resection of body of mandible with the primary tumor, with functional neck dissection lt side .

Male patient 84 years old presented with submandibular swelling firm ,CTScan reveals submandibular lymph node enlargement with central necrosis which is characteristic feature of malignant metastasis,also showing enlargement of parotid gland lt side .
True cut biopsy of submandibular lesion reveals mucoepidermoid carcinoma.
Planning by other surgeon to do total parotidectomy and sacrificing facial nerve with neck dissection.
PET scan showing increased SUV in submandibular lesion and small lesion of cheek .
For me I did good clinical examination and l found that tumor of mucosa of cheek firm mass which is showing increased SUV on PET scan,indicating malignant tumor of minor salivary gland which is the primary site,causing pressure and obstruction of duct of parotid gland causing secondary inflammation and increase in size of parotid gland which is realy not involved by malignant tumor.
So I just excised the cheek tumor and dissecting duct out with cannula and suturing it with doshioplasty technique to mucosa of cheek and preservation of parotid gland and facial nerve.
Also functional neck dissection done.
Surgery done by Dr.Sabah Abdulaziz/consultant of oral and maxillofacial surgery.
تم ولله الحمد اجراء عملية جراحية لرفع ورم خبيث من داخل الخد وتحرير قناة الغدة اللعابية للغدة النكافية واعادة تثبيتها داخل الفم مع الحفاظ على الغدة النكافية والعصب السابع التي كان المقرر رفعها من قبل جراح اخر .وتم استئصال كافة الغدد الليمفاوية من الرقبة والمحافظة على كافة الاعصاب والأعضاء الحيوية.
خرج المريض متحسنا ولله الحمد

75 years old woman presented with endophytic sq.cell carcinoma (moderately differentiated) of lateral side of tongue (T3) .PET scan shows no lymph nodes metastasis no distant metastasis ,sonography shows suspicious lymph node metastasis of level lB.
Clinically palpable submandibular lymph node.

Partial glossectomy with functional neck dissection
Removing all levels of lymph nodes from lev.l - lev.v.

Femal patient 12 years old presented with cystic ameloblastoma of lower jaw RT side , ramus and body , cripitus on palpation.
Opg and cbct showing mediolateral expansion and well defined radiolucent lesion , in certain area touching the periosteum.
Surgery done by resction of lesion from molar area and disarticulation of condyle due to extension of tumor.
Virtual planning already done with preparation of 3D model to prepare prebended reconstruction plate
,so alignment of autogenous bone graft from iliac region regarding specific dimensions to be fit on the plate .then applied to the defect region and fixed with body of mandible ,then tighten suture intra oral with good seal to separate oral cavity from bone graft.

تم واله الحمد اجراء عمليه جراحية مميزة لرفع ورم حميد كبير ادى الى تآكل الفك الاسفل حيث تم استئصال الورم من الفك من خلال بتر جزءمن الفك مع المفصل لاصابته بالورم وتم تعويضه بعظم طبيعي ذاتي من حوض المريضة وتم تثبيته ببليته معدنية معدة مسبقا من خلال التخطيط الافتراضي مع صناعة موديل .
وخرجت المريضة الطفلة ذات ١٢ عام بصحة جيدة ولله الحمد .
نتمنى الشفاء العاجل للمريضة.
