J-POP registry（院外心肺停止患者における近赤外線による無侵襲脳局所酸素飽和度【rSO2; Regional Cerebral Oxygen Saturation】測定の有用性に関する研究）：
“Chain of survival”に代表される心肺蘇生法により、OHCA患者の予後は大きく改善しています。また、近年、自己心拍再開後の“Post cardiac arrest syndrome”における低体温療法・経皮的人工心肺装置を用いた心肺蘇生法(ECPR; Extracorporeal Cardio Pulmonary Resuscitation)・緊急冠動脈形成術(PCI; Percutaneous Coronary Intervention)などの先端的治療法の実施により、OHCA患者に対し良好な臨床成績が認められたことが報告され、大きな注目を集めています。
一方では、“P

ost cardiac arrest syndrome”に対する医療の社会的・経済的な負担は極めて大きく、重大な社会問題となっています。しかしながら、OHCA患者への信頼性の高い予後予測法が無いため、“Post cardiac arrest syndrome”における先端的な治療法の適応は未だはっきりしておらず、このことがこれらの治療法の普及を遅らせていると指摘されています。近年発表された“Post cardiac arrest syndrome”に対する提言や心肺蘇生法に対するガイドラインにおいても、OHCA患者への診療という極めて緊急性の高い臨床現場における、即時性がありかつ信頼性の高い予後予測法の必要性が強調されています。
脳機能は酸素需要と供給のequivalentに依存し、低酸素血比率が上昇すると障害が進行することが知られています。脳における低酸素血比率は、低酸素血により多く吸収される近赤外線(NIRS)の特徴を利用し、rSO2値として、近赤外線分析装置(In Vivo Optical Spectroscopy: INVOS®)を用い前額部に貼布したセンサーから非侵襲的・リアルタイムかつ定量的に測定することが可能である。このようにrSO2値計測は即時的で侵襲性のない侵襲性のない脳循環評価法であり、現在まで主として心臓麻酔領域での脳保護目的のモニタリングとして用いられてきました。
このような背景のもと、今回、新たに多施設コホート観察研究を行い、OHCA患者におけるrSO2と神経学的予後との関連性を解析し、従来その予後予測が困難であったOHCA患者に対するrSO2計測の有用性を検討することを計画し、現在研究が進んでいます。

Brain protection therapy for patients with out-of-hospital cardiac arrest (OHCA) has greatly improved in recent years due to the development of emergency postcardiac arrest interventions for survival, e.g., mild therapeutic hypothermia, early percutaneous coronary intervention, and extracorporeal cardiopulmonary resuscitation (CPR).However, the indications for these costly interventions have not yet been established mainly because of difficulty in prognosticating neurological outcomes at hospital discharge on hospital arrival. Importantly, there is little time to consider whether or not highly intensive postcardiac arrest interventions should be performed. A prognostic index that can be evaluated on hospital arrival is needed to determine treatment strategies as quickly as possible after OHCA. Although many studies have identified factors associated with poor functional outcome after resuscitation, no study has shown a reliable predictor of neurological outcomes at hospital discharge. Previous reports have described that an inadequate intraoperative value of regional cerebral oxygen saturation (rSO2) is a significant predictor of postoperative neurological complications after cardiovascular surgery.11-14 rSO2 can be measured noninvasively in real time by near-infrared spectroscopy (NIRS). NIRS does not require vascular pulsation and can measure rSO2 even in patients with hypotension, hypothermia, and/or circulatory arrest. We hypothesized that neurological outcomes at hospital discharge in patients with OHCA can be predicted by the rSO2 values measured on hospital arrival. Therefore, we intended to validate the hypothesis in the present study.
