The Dorsalis Pedis Flap

For young surgeons, mastering the dorsalis pedis flap is a significant milestone in reconstructive orthopedics. This flap is valued for being thin, pliable, and possessing a reliable vascular anatomy. However, as with any delicate procedure, success lies in the details.

Indications & Rationale
Historically, this flap was a "workhorse" for various extremity defects. Modern perspectives, however, emphasize selectivity due to significant donor site morbidity:

Primary Indication: Today, its use is most justified when combined with second toe transfer for complex reconstructions.

Utility: It remains an excellent choice when you need to restore sensibility (neurovascular flap) or cover thin, pliable areas like the hand.

Key Surgical steps for Dissection

Anatomical Vigilance: The dorsalis pedis artery (DPA) is a continuation of the anterior tibial artery. Always verify DPA patency pre-operatively via palpation or Doppler.

Preserve the Fascia: Always include the deep fascia in your flap dissection to protect the pedicle and avoid separating it from the flap artery.

Dissection Plane: The flap is typically dissected disto-proximally. Meticulously preserve the paratenon of the extensor tendons to ensure a healthy bed for future coverage.

Handle with Care: Between the first and second metatarsal bones, visualize the first dorsal metatarsal artery carefully before proceeding.

Avoiding Complications

Donor Site Morbidity: This is the most common drawback. To mitigate this, avoid overly large flaps that necessitate complex skin grafting, which often leads to delayed healing and unacceptable cosmetic outcomes.

Venous Outflow: Insufficient venous drainage is a potential risk. Ensure the flap is not under excessive tension and that venous comitantes are carefully managed.

Distal Edge Planning: To prevent necrosis, keep the distal edge of the flap at least 2 cm proximal to the digital commissures.

Surgeon’s Note: While this flap offers unique advantages in tissue quality and sensory restoration, always weigh the "cost" of the donor site against the benefit to the patient. Meticulous technique is the only way to minimize the notorious donor site healing delays.
