💡Monkeypox is usually a self-limited disease with the symptoms lasting from 2 to 4 weeks. Severe cases can occur. In recent times, the case fatality ratio has been around 3–6%

💡Monkeypox is transmitted to humans through close contact with an infected person or animal, or with material contaminated with the virus.

💡Monkeypox virus is transmitted from one person to another by close contact with lesions, body fluids, respiratory droplets and contaminated materials such as bedding.

💡Monkeypox is a viral zoonotic disease that occurs primarily in tropical rainforest areas of central and west Africa and is occasionally exported to other regions.

💡An antiviral agent developed for the treatment of smallpox has also been licensed for the treatment of monkeypox.

💡Monkeypox is less contagious than smallpox and causes less severe illness.

💡Monkeypox typically presents clinically with fever, rash and swollen lymph nodes and may lead to a range of medical complications

Treatment of Frozen Shoulder

Frozen shoulder (also known as adhesive capsulitis) is a condition that causes pain and stiffness in the shoulder and can significantly interfere with daily activities.

Frozen shoulder typically has a sudden onset, often people literally wake up with it, and it is sharply painful to move with almost all shoulder movements. There may be some pre-warning signs such as the feeling a mild ache in the shoulders or some mild stiffness. It can also develop from a shoulder injury especially if you have been too afraid to move your shoulder after the injury.

It is suspected that an auto-immune reaction in the body causes joint capsule of the shoulder to literally “shrink wrap” the joint and a chemical reaction of inflammatory indicators invades the joint.

After the onset of frozen shoulder, it can be broken up into three phases; the freezing phase, the frozen phase, and the thawing phase.

Stage 1: Freezing
The “freezing” stage typically lasts from 6 weeks to 9 months. During this stage, pain increases and your shoulder loses range of motion. Sleeping can also be quite painful during this stage.

Stage 2: Frozen
The “frozen” stage typically lasts from 4 to 6 months. During this stage, painful symptoms may actually improve but stiffness remains. Daily activities may become very difficult to complete.

Stage 3: Thawing
During the “thawing” stage, shoulder motion will slowly improve and pain will be gone. It can take from 6 months to 2 years to return to normal strength and motion.

Physiotherapy after Total Knee Replacement (TKR)

Total Knee Replacement is a surgical procedure that replaces the existing knee joint with prosthetic components. The joint surfaces of the thigh bone (femur) and lower leg bone (tibia) are replaced with metal implants that cover the ends of the bone.

What Injuries/Conditions Cause a Total Knee Replacement?
Total knee replacement surgery is indicated (eg- Arthritis) when there is degeneration in the knee that causes pain, swelling and impaired function that did not respond to conservative treatments. Degeneration in the knee is commonly caused by arthritis. When the cartilage in the knee wears down and the joint surfaces are now carrying load, it will produce these symptoms, which will lead to functional limitations such as walking, stair climbing and even getting up from a seated position.

♦️How can Physiotherapy Help with Total Knee Replacements?

Physiotherapy will help with decreasing pain, regaining your range of motion and increasing your strength after surgery. Your physiotherapist will answer your questions and help you achieve your lifestyle goals. They also play an important part of ensuring the surgery is successful with achieving range and strength gains in the initial weeks post-op.

♦️What Should be Avoided with a Total Knee Replacement?

The surgery will help to diminish your pain and with physiotherapy you will re-gain your range of motion and strength to meet your daily functional demands. It is really important following surgery that you are seeing your physiotherapist regularly. However, you will need to avoid high impact activities (such as running or jumping) as that will accelerate the degradation of the prosthetic.

♦️How Long Does it Take to Recover from a Total Knee Replacement?
Recovery from a total knee replacement usually takes around 3-4 months of rehabilitation. It will be important that you maintain a level of exercise and weight management so you continue to have success with your new knee.

♦️What is Sciatica?

Sciatica is an umbrella term referring to all conditions or injuries that cause irritation to the sciatic nerve. The sciatic nerve is the largest nerve in the body. This nerve originates between the vertebrae in the low back and travels down the backside of each buttock, hip and leg.

♦️What are the Causes of Sciatica?

The sciatic nerve can be compromised in a variety of different ways. The nerve can be pinched or compressed by a herniated or pr*****ed disc and/or by a bone spur or narrowing of the inter vertebral foramen (a boney canal where the nerve exits the spine). The nerve can also be compressed through the myofascial (muscles and its connective tissue) tunnels that it passes through as it travels down your leg, such as the piriformis muscle.

♦️What are the Symptoms of Sciatica?

Since the nerve can become irritated in a variety of ways, the presentation can vary from person to person based on their unique presentation. In general, patients who have sciatic pain will complain of discomfort in their low back and leg. Also, patients may also describe feelings of shooting pain, tingling or numbness in their leg and foot as well as weakness of their leg muscles.

Symptoms that are more concerning is severe pain, tingling or numbness down both legs, increase frequency and urgency to urinate and constipation. If these symptoms are present it is urgent that you see a medical doctor that day.

♦️How can a Physiotherapist treat Sciatica?

Conservative management strategies such as physiotherapy or chiropractic care are treatment solutions that have been supported by the research and provide better outcomes over the long term. Your physiotherapist will complete a thorough evaluation of your back, pelvis, hips and lower extremity to determine the underlying cause of your sciatic symptoms.

Once your therapist determines where your sciatic nerve is being compressed they will use manual techniques to mobilize your restricted joints/tissues, use prescriptive exercise to improve your muscle imbalances and incorporate other modalities to facilitate recovery of the sciatic nerve. In addition, acupuncture and or dry needling/GunnIMS can be used to reduce your pain and facilitate nerve tissue healing.

Back pain is a common condition believed to effect four out of every five adults at some point during their lifetime.1 Mechanical back pain occurs when a structure in the back, like an inter vertebral disc, joint or a bone, is stressed or deformed in a way to cause pain. When a load is applied to a structure past a certain threshold, the tissue becomes irritated and increases pain. If the load is removed the pain will decrease.

Mechanical back pain can be localized to a small area on the spine or it can spread more to either one or both sides of the back. If a nerve root is also affected, the pain may radiate away from the back and down the leg, and even sometimes to the foot, along the path of the affected nerve.

▶️How do I get Mechanical Back Pain?

Mechanical back pain can occur from a trauma to the tissue, such as a fall or a car accident, or it can come on slowly over time with no obvious cause. Repetitive spinal movements, exercising incorrectly, and poor sitting habits are some examples that can lead to the development of mechanical back pain over time.

Physiotherapy can help to determine the root cause of the pain, and create a treatment plan to offload the injured tissue of the spine allowing for healing to occur. The treatment plan may include manual (hands-on) therapy, exercise therapy, and education about how to protect your back.

✳️Radiculopathy යනු කොඳු ඇට පෙළෙන් පිටවන ස්නායු මූලයන් එකක් හෝ වැඩි ගණනක සම්පීඩනය නිසා ඇතිවන තත්වයකි. ස්නායු තෙරපීම ශරීරයේ ක්‍රියාකාරීත්වයට බලපාන අතර තද ගතිය (tightness), වේදනාව(pain), හිරිවැටීම (numbness or tingling sensations) හෝ අත් පා දුර්වල වීමට හේතු වේ. ස්නායු තෙරපීම ඇති වන්නේ කොඳුඇටපෙළ අසලදී වන නමුත්, රෝග ලක්ෂණ බහුලව දක්නට ලැබෙන්නේ බලපෑමට ලක් වූ ස්නායු මූලයෙන් සපයනු ලබන නිශ්චිත ප්‍රදේශය තුළ ය.

▶️ස්නායු තෙරපීම බොහෝ විට සිදුවන්නේ බෙල්ලේ (cervical) සහ කොන්දේ(lumber region) ය. බෙල්ලේ ඇතිවන ස්නායු තෙරපීම් මගින් උරහිස් හෝ අත් වල රෝග ලක්ෂණ ඇති වන අතර කොන්දේ ඇතිවන ස්නායු තෙරපීම් මගින් බොහෝ විට පාද වල රෝග ලක්ෂණ ඇති වේ. රෝග ලක්ෂණ ශරීරයේ එක් පැත්තක හෝ දෙපැත්තෙන්ම ඇතිවිය හැකිය.
