🦴 “Not Just a Fracture: The Hidden Diagnosis Behind a Runner’s Hip Injury”

🏃‍♀️ Case Introduction

A 49-year-old recreational runner presented with persistent hip pain. She regularly participated in 10 km runs and had no history of trauma.

Initial imaging was subtle, but further assessment confirmed a femoral neck stress fracture — a high-risk injury that can lead to displacement if missed.

She underwent surgical fixation with good alignment.



🔩 Surgical Outcome

Post-operative imaging showed stable fixation of the femoral neck, allowing for controlled rehabilitation.

But the real question was:

👉 Why did this happen in an otherwise healthy runner?



🧪 The Clue Was in the Blood Tests

Investigations revealed:
• Low serum iron
• Very low transferrin saturation
• Microcytic red cell indices
• Slightly low total protein

These findings point toward:

🧠 The Real Diagnosis: RED-S

RED-S occurs when:

Energy intake is insufficient to meet the demands of exercise and normal body function

• Recovery is impaired

49 වයසැති කාන්තාවක්, සමාන්‍යයෙන් 10km දුර ධාවනය කරන අයෙක්, දිගුකාලීන hip වේදනාවක් සමඟ පැමිණියා. කිසිදු පැහැදිලි ආබාධයක් (trauma) නොමැතිව.

මුල් X-ray වලදී විශේෂ වෙනසක් නොපෙනුණත්, තවදුරටත් පරීක්ෂණයෙන් femoral neck stress fracture එකක් හඳුනාගන්න ලැබුණා — මෙය වැදගත් fracture එකක්, නිසිව හඳුනා නොගත්තොත් බරපතල ප්‍රතිවිපාක ඇතිවිය හැක.

ඇය සාර්ථකව ශල්‍යකර්මයක් මඟින් fix කරනු ලැබුවා.

Pain on the outside of your elbow?
Worse when lifting a kettle, writing, or typing?
You may have Tennis Elbow (Lateral Epicondylitis).

📌 What Is Tennis Elbow?

It is a tendon overload condition affecting the outer elbow.
⚠ It is NOT arthritis
⚠ It is NOT a fracture
⚠ It is NOT dangerous

It happens due to repeated strain on wrist extensor tendons.

Common in:
Teachers
Office workers
Manual workers
Homemakers

🔎 Try This Simple Test
✔ Straighten your elbow
✔ Make a fist
✔ Lift your wrist up against resistance
Pain at the outer elbow? → Likely tennis elbow.

🏥 What Should You Do?

1️⃣ Activity Modification
✔ Reduce repetitive gripping
✔ Avoid lifting with palm facing down
✔ Use two hands when lifting
✔ Take breaks every 30–40 minutes
❌ Do NOT completely rest the arm

බොහෝ වෙලාවට මේක “මස්පේශි ඇදීමක්” (muscle strain) විතරයි.
හරි විදිහට ව්‍යායාම කලොත් ආපහු ආපහු එන කොන්දේ වේදනාව අඩු කරගන්න පුළුවන්.

Groin Pain After Long-Distance Running: Don’t Ignore a Pelvic Stress Fracture

A 20-year-old female Air Force trainee presented with groin pain after completing a 10-mile marathon. She had normal hip movements but persistent pain during walking and running.
Her X-ray showed a subtle right inferior p***c ramus stress fracture.

This condition is more common than many people realize — especially in young endurance athletes.

What Is a P***c Ramus Stress Fracture?
The p***c ramus is part of the pelvic ring. During running, repetitive load passes from the spine to the pelvis and into the legs.

When:
Training intensity increases suddenly
Mileage increases rapidly
Recovery is inadequate
Small microscopic bone injuries accumulate. If loading continues, this progresses to a stress fracture.

Who Is at Risk?
Stress fractures of the pelvis are commonly seen in:
Military recruits
Marathon runners
Female endurance athletes
Individuals with sudden training escalation

In young women, we must also consider:
Low energy availability
Menstrual irregularities
Vitamin D deficiency
Inadequate calcium intake

Symptoms

Typical features include:
Deep groin pain
Pain worsened by running
Tenderness over the p***c bone
Usually normal hip range of motion
Importantly, hip joint movement is often normal — helping differentiate this from a femoral neck stress fracture.

Why Early Diagnosis Matters
Stress fractures are not “just muscle pain.”
If ignored:
Pain worsens
Healing is delayed
Risk of more serious fractures increases
Training interruption becomes longer
Early diagnosis allows safe recovery and structured return to sport.

Treatment Plan

Most inferior p***c ramus stress fractures are treated conservatively.

1. Relative Rest
Stop running immediately
Avoid impact activities
Walking as tolerated

2. Pain Control
Paracetamol preferred
Avoid excessive NSAID use

3. Address Risk Factors
Assess menstrual history
Check Vitamin D levels
Ensure adequate nutrition
Review training schedule

4. Gradual Return to Sport
Pain-free walking first
Then cross-training
Then progressive return to running over 4–6 weeks
Full recovery usually takes 6–8 weeks.

When to Seek Urgent Review

Immediate reassessment is required if:
Pain becomes severe
Pain develops in the hip joint
Difficulty weight-bearing
Symptoms fail to improve

This is important to exclude a femoral neck stress fracture, which requires more urgent intervention.

P***c Ramus Stress Fracture එකක් කියන්නේ මොකක්ද?
Pelvis හි p***c ramus කොටස මගින් ශරීර බර පාද වෙත සම්ප්‍රේෂණය වේ.
ඉතා වේගයෙන් (training intensity) එක දිගට කිරීම,
විවේක කාලය අඩු වීම,
දිගු දුර ධාවනය හදිසිව ආරම්භ කිරීම

දිවීම නවත්වන්න
Jumping / impact exercises වලින් වලකින්න
වේදනාව අනුව සීමිත ලෙස පයින් ගමන් කළ හැක

2️⃣ වේදනා නාශක
Paracetamol ප්‍රධාන වශයෙන් භාවිතා කරන්න
NSAIDs දිගුකාලීනව භාවිතා කිරීමෙන් වළකින්න

3️⃣ මූලික හේතු සොයා බැලීම
Vitamin D මට්ටම පරීක්ෂා කිරීම
Calcium intake සමාලෝචනය
Training schedule සකස් කිරීම
මාසික චක්‍රය සම්බන්ධ ඉතිහාසය විමසීම

4️⃣ ආපසු ක්‍රීඩාවට පැමිණීම
-පළමුව වේදනාව නොමැතිව ඇවිදීම
-පසුව low-impact exercises
4–6 සති තුළ gradual return to running
-සාමාන්‍යයෙන් සම්පූර්ණ සුව වීමට සති 6–8ක් පමණ ගතවේ.

දිගු දුර ධාවනයෙන් පසු groin වේදනාව “muscle strain” එකක් ලෙස නොසලකා හරින්න එපා.

🦴 Osteoporosis: The Silent Disease Behind Fragile Fractures

Many patients come to us with a wrist fracture after a simple fall.
They fix the fracture… but forget to fix the real problem — osteoporosis.

What is Osteoporosis?
Osteoporosis is a condition where bones become:
Thin
Weak
Fragile
This makes them break easily — sometimes even after a minor fall.
It is often called the “silent disease” because there are no symptoms until a fracture happens.

Common Fractures Due to Osteoporosis
The most common fractures include:
Wrist fractures (distal radius fractures)
Hip fractures
Spine (vertebral compression fractures)
A wrist fracture in a woman over 50 is often the first warning sign of osteoporosis.

Who Is at Risk?

You may be at higher risk if you:
Are above 50 years of age
Are postmenopausal
Have rheumatoid arthritis
Take long-term steroids
Have a family history of fractures
Have low body weight
Smoke or drink excess alcohol

⚠️ Patients with rheumatoid arthritis have a higher risk due to:
Chronic inflammation
Steroid use
Reduced physical activity

Why Is It Important to Treat Osteoporosis?
Fixing the fracture is not enough.
If osteoporosis is untreated:
The risk of hip fracture increases
Spine fractures can cause height loss and back pain
Future fractures become more likely
A hip fracture in elderly patients can significantly affect independence and quality of life.

How Do We Diagnose Osteoporosis?
The gold standard test is:
📊 DEXA Scan (Bone Density Scan)
It measures bone mineral density and gives a T-score:
Normal: Above -1
Osteopenia: -1 to -2.5
Osteoporosis: Below -2.5
Treatment of Osteoporosis

Treatment usually includes:

1️⃣ Lifestyle Changes
Regular weight-bearing exercise
Adequate sunlight exposure
Stop smoking
Reduce alcohol intake

2️⃣ Calcium & Vitamin D
Calcium supplements
Vitamin D supplementation

3️⃣ Medications
Depending on risk level:
Bisphosphonates (e.g., Alendronate)
Denosumab
Teriparatide (in severe cases)

Treatment choice depends on:
Age
Fracture history
Bone density score
Kidney function

# # **Anterior Shoulder Pain – Is It the Biceps Tendon?**

Many patients come to the clinic with **pain in the front of the shoulder**, especially when lifting the arm, reaching overhead, or trying to touch the head or back.
A common cause of this pain is **biceps tendon irritation**, also known as **Biceps Tendinitis** or **Biceps Tendinopathy**.

# # **What is the Biceps Tendon?**

The biceps muscle has **two tendons at the shoulder**.
One of them, called the **long head of the biceps tendon**, runs through a groove at the front of the shoulder and helps with:

* Lifting the arm
* Rotating the shoulder
* Stabilising the shoulder joint

Because of its position, this tendon is **prone to irritation and inflammation**, especially with repeated use.

# # **How Do Patients Usually Present?**

Typical symptoms include:

🔹 Pain at the **front (anterior) of the shoulder**
🔹 Pain when **lifting the arm**, especially between certain angles
🔹 Difficulty or pain when:

* Touching the head
* Reaching behind the back
* Overhead activities

🔹 Pain increases with:

* Lifting objects
* Carrying bags
* Repetitive arm use

🔹 **Tenderness when pressing over the biceps tendon** (front of shoulder)

Sometimes patients say:

> “Doctor, it hurts at a certain point when I lift my arm.”

Common causes include:
