如果咳嗽持续超过 8 个星期,除了感染、吸烟或肺部疾病之外,还有一个常常被忽略的原因——长期暴露于家中的刺激物或过敏原。

家里隐藏的慢性咳嗽元凶:
- 枕头、床垫、窗帘和毛绒玩具里的尘螨. 尘螨是全球最常见的室内过敏原之一。它们的排泄物和残骸非常微小,容易随着空气飘散,被吸入后可能引起气道发炎和咳嗽。如果咳嗽在夜间或清晨特别明显,更需要考虑这个原因。
- 霉菌和潮湿环境. 霉菌会释放大量微小孢子到空气中,容易刺激呼吸道,引发咳嗽、喘鸣,甚至诱发哮喘发作。浴室、冷气机内部以及通风不良的房间,都是常见的藏身之处。
- 肮脏的冷气过滤网. 冷气本身不会制造灰尘,但如果过滤网长期没有清洗,就可能积聚灰尘、霉菌和过敏原,并随着冷气运转不断在室内循环。
- 烹饪油烟. 高温煎炒时会产生大量细悬浮微粒,也就是大家常听到的 PM2.5。这些微粒非常细小,足以深入肺部,长期暴露可能导致慢性咳嗽及呼吸道症状恶化。
- 蚊香、香烛和线香. 许多人不知道,燃烧一片蚊香所产生的悬浮微粒,可能相当于数十支香烟所产生的量。而香烛和线香燃烧时,也会释放细微颗粒和刺激性化学物质,对肺部并不友善。
- 香味产品: 空气清新剂、香薰蜡烛、精油和香水等产品,会释放挥发性有机化合物(VOC)。对于呼吸道较敏感的人来说,这些气味可能诱发咳嗽或喉咙不适。
- 宠物: 真正的问题通常不是毛发本身,而是宠物的皮屑、唾液和尿液中的蛋白质。这些过敏原可以长时间悬浮在空气中
- 二手烟与三手烟: 即使家里没人吸烟,烟雾中的有害物质仍可能残留在衣服、窗帘、沙发和地毯上,持续数周甚至数月,这就是所谓的“三手烟”。

以下情况,提示家里的环境可能与咳嗽有关:
咳嗽在夜间或清晨特别严重, 外出旅行或住在其他地方时症状明显改善, 家人本身有过敏或哮喘病史, 打扫卫生、整理床铺或进入某些房间时特别容易咳嗽.

If your cough has lasted more than 8 weeks, one possible explanation is long-term exposure to irritants or allergens inside the house.

Hidden causes of chronic cough inside the house:

- Dust mites in pillows, mattresses, curtains and soft toys: Dust mites are among the most common indoor allergens worldwide. Their microscopic waste particles can trigger airway inflammation and coughing, especially if symptoms are worse at night or early morning.
- Mould and damp areas: Mould releases tiny airborne spores that can irritate the airways and trigger coughing, wheezing and asthma attacks in susceptible individuals. Bathrooms, air-conditioners and poorly ventilated rooms are common hiding places.
- Dirty air-conditioner filters: Air-conditioners do not create dust, but poorly maintained filters can accumulate dust, mould and allergens, which are then continuously recirculated into the room.
- Cooking fumes: Cooking, especially frying at high temperatures, produces very fine particles known as PM2.5. These particles are small enough to travel deep into the lungs and can worsen cough and respiratory symptoms over time.
- Incense, joss sticks and mosquito coils: Many people are surprised to learn that burning one mosquito coil can produce particulate matter comparable to dozens of ci******es, while incense smoke also contains fine particles and irritant chemicals that affect the lungs.
- Scented products: Air fresheners, scented candles, essential oils and perfumes release volatile organic compounds (VOCs), which may trigger coughing or throat irritation in sensitive individuals.
- Pets: The problem is usually not the fur itself. Many people react to proteins found in pet saliva, skin flakes (dander) and urine, which can remain airborne for long periods.
- Second-hand and third-hand smoke: Even if nobody smokes inside the house, smoke particles can remain on clothes, curtains, sofas and carpets for weeks or months. This is known as third-hand smoke.

Some clues that your home environment may be contributing to your cough: Cough is worse at night or early morning
Symptoms improve when travelling or staying elsewhere. Family members have allergies or asthma
Cough is triggered by cleaning, making the bed, or entering certain rooms

Considering that an average person takes around 20,000 breaths a day, even small exposures can matter when repeated over many years.

这一周,我诊断了 3 位肺结核患者。
他们都很年轻,最小的只有 12 岁。
很多人以为,肺结核只是上一代才会得的病,现代已经很少见了。
其实,肺结核至今仍然存在,而且并不少见。

根据卫生部数据,马来西亚在 2024 年共通报 26,183 宗结核病(TB)病例,平均几乎每小时就有 3 人被确诊肺结核。而且这个号码正在逐年上升.
这只是已经被诊断和通报的病例。(Notified cases)
实际上,仍然存在一些尚未被发现或尚未确诊(undiagnosed)的结核病患者。他们可能因为症状轻微、误以为只是普通咳嗽,或迟迟没有求医,而继续在社区中传播疾病。
这也是为什么结核病至今仍未绝迹。

常听到病人说:
「我朋友有肺结核,但我只是偶尔见到他,应该不会被传染吧?」
肺结核是经由空气传播的疾病。咳嗽、说话、唱歌时,都可能把带有结核菌的飞沫微粒释放到空气中。虽然与患者共同生活的人风险最高,但只要曾与具有传染性的患者有较长时间接触,尤其是在空气不流通的环境,仍然有机会受到感染。
如果曾接触过肺结核患者,请不要因为自己没有症状,就忽略筛检。
感染结核菌后,有些人会马上发病,但也有人会潜伏多年,之后才发展成活动性肺结核。

肺结核是可以治愈的。
关键在于及早发现、及早诊断、及早治疗。

This week alone, I diagnosed three patients with tuberculosis (TB)

They were all young, and the youngest was only 12 years old.

Many people believe that tuberculosis is a disease of the past—that it mainly affected the older generation and is now rarely seen.

Unfortunately, that is not the case.

Tuberculosis is still very much with us, and it remains a significant public health problem.

According to Malaysia's Ministry of Health, 26,183 TB cases were officially notified in 2024, an average of almost three people diagnosed every hour. The number has also been increasing over the years.

And these are only the notified (diagnosed and reported) cases.

In reality, there are likely individuals with undiagnosed TB in the community. Some have only mild symptoms, some mistake their persistent cough for a common cold or allergies, and others delay seeking medical attention. During this time, they may continue to spread the infection unknowingly.

This is one of the reasons why tuberculosis has not been eliminated.

I often hear patients say:

"My friend had TB, but I only met them occasionally. Surely I wouldn't have caught it?"

Tuberculosis spreads through the air. When a person with infectious pulmonary TB coughs, speaks, sings, or even breathes, tiny droplets containing the bacteria can remain suspended in the air. While household contacts are at the highest risk, prolonged exposure in poorly ventilated spaces—even outside the home—can also result in infection.

If you have been identified as a close contact of someone with TB, please attend the recommended screening, even if you feel completely well.

After infection, some people develop active TB soon afterwards, while others may carry the bacteria silently for months or even years before becoming ill.

The good news is that tuberculosis is curable.

The key is early recognition, early diagnosis, and early treatment.

这是许多特发性肺纤维化(Idiopathic Pulmonary Fibrosis,IPF)患者最常说的话, 是肺纤维化逐渐进展的表现。

IPF 是一种慢性、进行性的肺部疾病。简单来说,肺部组织会逐渐出现疤痕化(纤维化),使肺脏变得越来越硬,失去原有的弹性。随着时间推移,肺部吸收氧气的能力下降,患者会越来越容易气喘、疲倦,活动能力也逐渐受限。
许多人在疾病早期并没有明显不适,只是发现自己爬楼梯比较喘、走路跟不上同龄人,或者持续干咳数个月甚至数年。因此,不少患者误以为这是年龄增长、缺乏运动或体力变差所导致,而延误了诊断。

目前医学界仍未完全了解 IPF 的确切成因,因此被称为特发性。它通常发生在 50 岁以上人士,男性较为常见,吸烟史也是其中一个风险因素。
过去,IPF 被认为是一种无法治疗的疾病。然而近年来,随着抗纤维化药物(Antifibrotic Therapy)的出现,治疗观念已经发生改变。这类药物虽然无法逆转已经形成的疤痕,但能够减缓肺功能恶化的速度,帮助患者保留更多肺功能。
因此,及早诊断非常重要。
因为对于肺纤维化患者而言,时间就是肺功能。
越早发现,越早介入治疗,就越有机会延缓疾病进展,维持生活品质。

"I used to be able to walk around the entire shopping mall. Now I need to stop and rest halfway."
"I've never coughed for this long before. This cough has been going on for six months."
These are common stories I hear from patients diagnosed with Idiopathic Pulmonary Fibrosis (IPF), a condition in which scarring gradually develops within the lungs.
IPF is a chronic and progressive lung disease. Put simply, the lungs slowly become scarred and stiff, losing their normal elasticity. As this scarring worsens, the lungs become less efficient at transferring oxygen into the bloodstream. Patients may notice increasing breathlessness, fatigue, and a gradual decline in their ability to carry out everyday activities.
In the early stages, symptoms can be subtle. Many people simply find themselves becoming more breathless when climbing stairs, unable to keep up with their peers during a walk, or troubled by a persistent dry cough that lasts for months. As a result, these symptoms are often dismissed as ageing, lack of fitness, or simply "getting older," leading to delays in diagnosis.
The exact cause of IPF remains unknown, which is why it is called idiopathic. It most commonly affects individuals over the age of 50, is more frequently seen in men, and is associated with risk factors such as a history of smoking.
Not long ago, IPF was considered a disease with very limited treatment options. Today, however, the availability of antifibrotic therapy has changed the way we manage this condition. While these medications cannot reverse existing scarring, they can help slow the progression of lung function decline and preserve lung capacity for a longer period.
This is why early diagnosis matters.
For patients living with pulmonary fibrosis, time is lung function.
The earlier the condition is recognised and treated, the greater the opportunity to slow disease progression and maintain quality of life.

最近让我印象深刻的是一位哮喘病人。

她患有哮喘多年。几年前一次肺感染后,病情开始明显恶化。她经常咳嗽、咳痰、气喘,白天会发作,晚上也会因为喘或液咳而醒来。
她几乎每天都需要使用缓解型吸入器(Rescue Inhaler)。
单单去年她就因为哮喘发作而多次反复住院。每次出院后情况稍微改善,但不久后又再次发作。

当她来到我的门诊时,其实已经接受了国际哮喘治疗指南(GINA)中的第五级(Step 5)治疗,也就是目前吸入药物治疗中最强化的吸入器和口服治疗方案。尽管如此,她的哮喘依然控制不佳,白天和夜间症状持续存在,并且反复因为急性发作而住院。由于她每日往返新加坡工作,频密的病假已影响了她的生活.

经过进一步评估后,发现她属于重症哮喘(Severe Asthma),而且是其中一种称为“第二型发炎(Type 2 Inflammation)”的哮喘表型。这类病人的问题不仅仅是气管敏感,而是免疫系统中的某些发炎途径持续被激活。(5-10%的哮喘病患属于重症哮喘)

根据国际哮喘治疗指南(GINA)以及她的临床特征,我开始用生物制剂(Biologic Therapy)为她治疗。
生物制剂并不是传统的类固醇,也不是另一种吸入器。它是一种针对特定免疫发炎途径的精准治疗,通过皮下注射方式给药,通常每 2 至 8 周注射一次, 抑制引发哮喘发炎的错误免疫讯号,从源头减少发炎反应,进而降低哮喘发作和住院的风险。

两个月后,变化令人鼓舞。她不再因为哮喘住院。夜间症状明显减少。吸入器剂量成功降低,也不再需要反复依赖口服类固醇来控制病情。
当然,并不是每一位哮喘患者都需要生物制剂。

事实上,大多数哮喘患者通过规律使用吸入器便能获得良好控制。生物制剂主要适用于经过专科全面的评估后,仍然控制不佳的严重哮喘患者。
但对于严重哮喘患者而言,生物制剂不仅是一种新药,更代表着哮喘治疗的一场重要变革,从控制症状,逐步迈向精准治疗。

过去我们只能不断增加药物剂量来压制症状;今天,我们已经能够针对特定发炎机制进行更精准的治疗。

对于严重哮喘患者而言,这是重新拥有正常生活的机会。

Many people think asthma is a condition that can be controlled simply by using an inhaler.
For most patients, that is true.
But for a small group of people living with severe asthma, the reality can be very different.
I recently met a lady who had been struggling with asthma for years. Following a serious chest infection several years ago, her symptoms gradually became worse. She was coughing daily, bringing up phlegm, feeling breathless, and waking up at night because of coughing and wheezing.
Her rescue inhaler had become part of her daily routine.
In the past year alone, she had been admitted to hospital multiple times because of asthma attacks. Each admission would bring temporary relief, but before long, the symptoms would return.
By the time she came to see me, she was already receiving the highest level of standard asthma treatment recommended by international guidelines (GINA step 5 treatment). Despite multiple inhalers and medications, her asthma remained poorly controlled. Frequent sick leave was beginning to affect both her work and daily life.
Further assessment showed that she belonged to a subgroup known as severe Type 2 asthma. In these patients, the problem is not simply sensitive airways. Instead, certain inflammatory signals within the immune system remain persistently overactive.
This is where biologic therapy may play a role.
Unlike inhalers, biologics are targeted treatments given as injections every few weeks. They work by blocking specific inflammatory pathways that drive asthma, helping to reduce attacks, hospitalisations, and the need for repeated steroid treatment.
Within two months of starting treatment, the change was encouraging. She had not required another hospital admission. Her night-time symptoms improved significantly, and we were able to reduce the intensity of her asthma medications.
Of course, biologics are not for everyone. Most asthma patients achieve excellent control with inhalers alone. However, for carefully selected patients with severe asthma, biologics represent an important shift in treatment—from simply controlling symptoms to targeting the underlying disease process.
For those whose lives have been repeatedly disrupted by severe asthma, that shift can mean much more than better breathing.
It can mean getting their lives back.

上周,陈老先生在女儿的强迫下来到了我的诊间。一坐下来,他就摆摆手,喘着气对女儿抱怨道:“都说不用来看医生啦,人老了就是这样的嘛,走几步路不够气,正常的啦!”陈伯伯今年68岁,是一位有着三十年烟龄的老烟枪。这几年,他开始发现自己有些不对劲。以前走上三楼找邻居喝茶不费吹灰之力,现在走平路稍微快一点,胸口就闷得慌;早上起床,总要剧烈咳嗽一顿,咳出几口白痰才觉得舒服。他一直以为这只是老了、是抽烟咳,于是他开始变得不出门、不运动,每天就坐在家里看电视。
但实际上,这些正是慢阻肺(COPD)的典型早期信号。
为他做了CT和肺功能测试,确诊慢阻肺(或肺气肿), COPD.
慢阻肺的早期症状非常隐蔽,主要表现为长期慢性咳嗽、咳痰, 以及活动后气喘。很多患者在爬楼梯、提重物,甚至洗澡、穿衣服时觉得不够气,却像陈老先生一样,总觉得老了不中用是正常的,从而错过了最佳的干预时机。
那么,一旦确诊了该怎么办呢?虽然被破坏的肺泡无法完全复原,但通过科学的医学介入,我们完全可以做到止损并提高生活质量。
首先,戒烟是最重要最有效的第一步,任何时候戒烟都不晚。
其次,必须遵医嘱规律使用医生开具的吸入剂,缓解气喘。
最后,在专业指导下进行肺康复运动,如缩唇呼吸、腹式呼吸训练以及适度的体力活动,能让剩下的肺发挥最大的功能,重获自由呼吸。

Last week, Mr. Chen came into my clinic, practically forced by his daughter. The moment he sat down, he waved his hand and complained to her out of breath, "I told you there's no need to see a doctor! This is just part of getting old. Feeling short of breath after walking a few steps is completely normal!" Mr. Chen is 68 years old and a heavy smoker with a 30-year history. Over the past few years, he started noticing that something wasn't quite right. Climbing up to the third floor to have tea with his neighbors used to be effortless, but now, even walking a bit faster on flat ground makes his chest feel tight. Every morning when he gets out of bed, he goes through a severe coughing fit, spitting up a few mouthfuls of white phlegm before feeling some relief. Believing this was just old age or a smoker's cough, he stopped going out, gave up exercising, and spent his days just sitting at home watching TV.
In reality, these are textbook early warning signs of COPD (Chronic Obstructive Pulmonary Disease). The early symptoms of COPD are incredibly stealthy, primarily presenting as a chronic, long-term cough, phlegm production, and shortness of breath during exertion. Many patients feel winded while climbing stairs, lifting heavy objects, or even during basic daily activities like bathing and dressing. Yet, just like Mr. Chen, they dismiss it as simply getting older and losing their physical edge, thereby missing the best window for medical intervention.
So, what should you do if you are diagnosed? Although damaged alveoli (lung air sacs) cannot be fully restored, scientific medical intervention can effectively stop further damage and significantly improve your quality of life. First and foremost, quitting smoking is the most crucial and effective step—it is never too late to quit. Second, you must consistently use the inhalers prescribed by your doctor to relieve breathlessness. Lastly, participating in pulmonary rehabilitation under professional guidance—such as learning pursed-lip breathing, diaphragmatic breathing, and engaging in moderate physical activity—can help your remaining lung capacity function at its absolute best, allowing you to reclaim the freedom of breathing.

年刚过,好多病人就因为咳嗽而就诊, 其实这和新年期间的一些生活习惯有关。

农历新年大家都会吃很多年饼、肉干,花生和甜甜或油炸的零食,这些食物比较燥,容易刺激喉咙,让人更容易咳嗽。
另外,杆虽然是新年的吉祥水果,但如果吃太多,酸性也可能让喉咙有生痰而不舒服。
新年期间燃放鞭炮和烟花,虽然增加了新年气氛,但空气中的烟雾和细小颗粒会刺激呼吸道,气道敏感的人可能会咳嗽至气管收缩而呼吸困难。
今年新年的头几天刚好下雨,天气冷又潮湿,呼吸道也会变得比较敏感。
再加上拜年、熬夜和饮食不规律,身体抵抗力稍微下降,所以更容易出现咳嗽。
一般来说,多喝温水、减少零食、好好休息,通常会慢慢好转。
不过,如果咳嗽持续超过2-3个星期,或者伴随发烧、气喘、痰多或带血,就一定要尽早就医检查。

病从浅中医。

Just after the Chinese New Year, many patients come to the clinic because of cough. In fact, this is often related to some lifestyle habits during the festive season.

During the Lunar New Year, people tend to eat a lot of festive snacks such as cookies, barbecued meat, peanuts, and sweet or fried foods. These foods are considered “heaty” and can irritate the throat, making coughing more likely. Oranges, although a symbolic fruit for prosperity during the New Year, can also cause throat irritation or increased phlegm if eaten in excess. In addition, the use of firecrackers and fireworks, while adding to the festive atmosphere, releases smoke and fine particles into the air that can irritate the airways. People with sensitive airways may cough or even experience airway tightening that leads to breathing difficulty.

This year, the first few days of the New Year were also rainy, with cooler and more humid weather, which can make the respiratory tract more sensitive. Together with visiting relatives, staying up late, and irregular eating habits, the body’s immunity may temporarily decrease, making cough more likely.

In most cases, drinking warm water, reducing snacks, and getting adequate rest will allow the cough to gradually improve. However, if the cough lasts longer than two to three weeks, or is accompanied by fever, wheezing, excessive phlegm, or blood in the sputum, it is important to seek medical attention early.

A small reminder: Over the past few days in clinic and during ward rounds, I’ve probably said this a hundred times or more. The smoke may trigger or worsen respiratory symptoms.

最近很多朋友突然私讯我,问我关于新山和笨珍最近出现的肺结核(TB)病例。

其实, TB 并不是突然出现的新病,它一直都存在于马来西亚,只是最近因为出现较多病例、加上媒体报道,大家才开始特别关注。

日常看诊时也经常遇到 TB 病例,甚至有些病人完全没有症状,是在申请新加坡工作准证时做体检胸部X光(CXR),结果发现影像异常,进一步检查后才意外诊断出 TB。很多人听到都会很惊讶,因为他们平时没有咳嗽、没有发烧,也没有不舒服。这也说明 TB 在早期其实可以非常隐匿,甚至完全没有症状,但已经有感染的可能。

TB 是一种由结核杆菌引起的传染病,最常影响肺部,所以很多人会出现长期咳嗽的情况。它主要通过空气传播,比如患者咳嗽、打喷嚏或讲话时,把细菌释放到空气中。如果长期在密闭、不通风的环境和患者近距离接触,就有可能被感染。

需要注意的是,如果咳嗽超过两到三个星期还没好,或者伴随发烧、夜间盗汗、体重下降、食欲变差、疲倦,甚至咳血,这些都不应该忽略,尽早去检查。医生通常会安排胸部X光和痰液检查,有些情况也可能需要更进一步的检查。

真的中了怎么办? TB 是可以治疗, 也可以治愈的疾病。只是疗程比较长,需要至少六个月,而且一定要按时吃药,不能觉得好一点就停药。因为停药或吃药不规律,会增加抗药性TB的风险,到时治疗会更困难,时间也会更长。

预防方面,最重要的是保持环境通风,尤其家里或办公室多开窗、让空气流通。在人多的地方,戴口罩保护自己保护家人也是很重要的。另外,免疫力较弱的人(例如糖尿病患者、长期服用类固醇的人、老人)更需要提高警觉。若家中有人确诊TB,密切接触者应该尽早做筛查,这样才能避免传播给其他家人。

Recently, many friends have been messaging me about the TB (tuberculosis) cases reported in Johor Bahru and Pontian.

Just to share, TB is not a new disease. It has always existed in Malaysia. It’s just that recently, with more cases being detected and highlighted in the media, people are paying more attention to it.

In fact, in my daily practice, I still come across TB cases quite often. Some patients have no symptoms at all, and their TB was only discovered incidentally during a routine chest X-ray (CXR) for Singapore work permit medical screening. Many are surprised because they feel completely well. This shows that TB can be silent in the early stage, even though infection is already present.

TB is caused by a bacteria called Mycobacterium tuberculosis and most commonly affects the lungs. It spreads through the air when an infected person coughs, sneezes, or talks, especially in enclosed and poorly ventilated environments with prolonged close contact.
Please seek medical attention if you have a cough lasting more than 2–3 weeks, especially if accompanied by fever, night sweats, weight loss, poor appetite, tiredness, or coughing up blood. Doctors usually arrange a chest X-ray and sputum tests for diagnosis.

The good news is TB is treatable and curable. However, treatment usually requires at least 6 months, and medication must be taken consistently. Stopping early or irregular treatment can lead to drug-resistant TB, which is much harder to manage.

For prevention, good ventilation is important. Keep windows open and allow air circulation. Wearing a mask in crowded places also helps protect yourself and your family. Those with weaker immunity (such as diabetes, elderly, or long-term steroid use) should be extra cautious. If a household member is diagnosed with TB, close contacts should go for early screening.
