Rotational Angular Breathing (RAB) is a special breathing technique used in Schroth-based scoliosis exercises to de-rotate the spine, expand collapses area of the rib cage and improve posture in scoliosis patient.

Rotational Augular breathing (RAB) teaches us to direct our breathing toward these areas (concave side) to encourage expansion and movement of the rib cage.

🫁 In simple terms: We learn to “breathe into” the areas that need more space.

Most importantly, the breathing technique is tailored to each person's individual scoliosis curve, because every curve is different.

Case Progress: Scoliosis Management Success

A 15-year-old patient first presented to our physiotherapy centre with an initial Cobb angle of 25°

Following a comprehensive conservative treatment plan, the patient was prescribed a custom scoliosis brace and participated in a Schroth-based physiotherapy program.

After one year of consistent brace wear and dedicated Schroth rehabilitation, a follow-up X-ray demonstrated a significant improvement. The patient's Cobb angle reduced from 25° to 14°, representing an 11° reduction in spinal curvature.

This case highlights the importance of early detection, individualized treatment, consistent brace compliance, and scoliosis-specific exercises in the conservative management of adolescent idiopathic scoliosis. While individual outcomes vary, this patient's progress demonstrates that, with commitment and appropriate clinical guidance, meaningful improvements in spinal alignment can be achieved without surgery.

Sleeping Recommendations for Patients with Double-Curve Scoliosis

Patients with double-curve scoliosis generally cannot be given a single sleeping recommendation because the two major curves oppose each other, making it difficult to recommend one sleeping position for everyone.

However, if the major curve is in the lumbar spine, sleeping on the concave side may be beneficial.

Example: Left Lumbar Curve

If you have a left lumbar curve, your right side is the concave side. Therefore, you are encouraged to sleep on your right side on a firm mattress.

When you sleep on your concave side (right side):

✅The convex side is allowed to gently move toward the body's midline
✅The concave side is gently stretched under the effect of gravity.
✅This position helps promote a more balanced and aligned spinal posture during rest.

Lying Position for a Major Thoracic Curve.

If you have a right thoracic curve, the curve bulges out (Convex) on the right side and curves inward (Concave) on the left side.

It is recommended to lie on your left side (the concave side) on a soft bed.

When you lie on your left side:

✅The right side of your rib cage (the bulging side) can gently relax and move toward the middle.
✅The left side (the hollow side) can gently stretch open.

The aim is not to force you to sleep in this position all night. Instead, try to get used to this position when you are resting, reading, or relaxing in bed. Over time, you may naturally adopt this position while sleeping.

Scoliosis isn't always visible, and wearing a brace is more than just putting on a device - it's part of a journey toward protecting a growing spine. Every hour spent wearing a brace is an investment in long-term spinal health.

Wearing a scoliosis brace can take time to get used to, both physically and emotionally. Many people find that confidence grows as the brace becomes part of their routine. Here are some practical tips:
1. Follow your specialist recommended wearing time
2. Dress comfortably
3. It's normal to have mild redness. If you develop blisters, contact your brace specialist
4. Keep the brace clean by wiping the brace and let it dry before putting it back on.
