Rotator cuff tears are one of the most common causes of shoulder pain and weakness, particularly in adults over 40.

Contrary to popular belief, a rotator cuff tear does not mean the shoulder muscles have torn apart. In most cases, it is the tendon—most commonly the supraspinatus tendon—that becomes partially or completely torn where it attaches to the bone.

Rotator cuff tears may develop gradually due to age-related tendon degeneration or occur suddenly following trauma such as a fall. Common symptoms include pain when lifting the arm, weakness, difficulty performing overhead activities, and night pain that interferes with sleep.

Not every tear requires surgery. Treatment depends on several factors, including:
• Size and location of the tear
• Age and activity level
• Severity of symptoms
• Functional limitations
• Response to conservative treatment

Many partial-thickness tears and selected full-thickness tears can be managed successfully with appropriate rehabilitation, while larger or traumatic tears may require surgical repair.

An accurate diagnosis is essential to determine the most appropriate treatment and achieve the best possible outcome.

**Shoulder pain isn't always a rotator cuff injury. Sometimes, the problem lies in the shoulder's "shock absorber"—the labrum.**

The **labrum** is a ring of fibrocartilage that deepens the shoulder socket and helps keep the joint stable. It can become **frayed, chafed, partially torn, completely torn, or detached**, with **SLAP lesions** being one of the more common injuries affecting the upper labrum.

Because the labrum has a **limited blood supply**, healing can be slower. However, **not every labral injury requires surgery**. Many people improve with an accurate diagnosis, appropriate rehabilitation, and activity modification.

**Common symptoms:**
✔ Deep shoulder pain
✔ Clicking or catching sensations
✔ Pain during overhead movements
✔ Feeling of instability or weakness

**Remember:** A shoulder click doesn't always mean a labral tear, and not every labral tear requires surgery. The right diagnosis is the first step towards the right treatment.

**Most shoulder pain is NOT caused by calcific tendinitis—but when it does occur, it can be one of the most painful shoulder conditions.**

Calcific tendinitis occurs when **calcium hydroxyapatite crystals** form within a rotator cuff tendon, most commonly the **supraspinatus tendon**. Interestingly, many people have these deposits without any symptoms.

Pain usually develops **not because the calcium is there**, but when the body starts **breaking down (resorbing)** the deposit. This inflammatory phase can cause sudden, severe shoulder pain, making it difficult to lift the arm, get dressed, or sleep on the affected side.

**The facts:**
✔ There is no strong evidence that dietary calcium or calcium supplements cause calcific tendinitis.
✔ The calcium deposit is inside a **tendon**, not the shoulder joint.
✔ Although the symptoms may resemble a rotator cuff tear, they are different conditions and require different management.

Diagnosis is based on a detailed clinical assessment and is often confirmed with **X-rays**, while ultrasound may be used to further evaluate the deposit or guide treatment.

Most people recover with **conservative care**. Chiropractic management may help improve shoulder mobility, reduce pain, and restore normal shoulder mechanics. However, **a structured rehabilitation programme remains the cornerstone of recovery**, helping restore strength, function, and reducing the likelihood of recurrent symptoms.

**Pain on the top of your shoulder? It might not be your rotator cuff.**

Pain that is **localized on the top of the shoulder** may be coming from the **Acromioclavicular (AC) Joint**—the small joint where your collarbone meets your shoulder blade. This is a common source of shoulder pain, particularly in people who perform repetitive overhead activities, heavy lifting, weight training, or have had a previous fall onto the shoulder.

One of the key clinical clues is **pain when reaching across your body**, such as fastening a seatbelt, washing the opposite shoulder, or performing bench presses and push-ups. Many people can also point to the painful area with a single finger directly over the top of the shoulder.

Although these symptoms may suggest AC joint involvement, **they are not enough to make the diagnosis**. Other conditions, including rotator cuff disorders, shoulder impingement, and referred neck pain, can present in a similar way. A thorough assessment is essential to identify the true cause.

**Frozen Shoulder isn't just a stiff shoulder.**

Many people are diagnosed with a "frozen shoulder" when the real problem is a rotator cuff injury, shoulder impingement, or a biceps tendon disorder.

True **Frozen Shoulder (Adhesive Capsulitis)** affects the **joint capsule**, causing it to become inflamed, thickened, and contracted. One of its hallmark signs is that **both active and passive shoulder movements become restricted**, especially external rotation.

Unlike many other shoulder conditions, **no single adjustment or manipulation can reverse a frozen shoulder.** Recovery usually requires an accurate diagnosis, guided rehabilitation, pain management, and time.

Shoulder pain can arise from many different structures, including the rotator cuff, long head of the biceps tendon, subacromial bursa, shoulder joint, AC joint, or even be referred from the neck.

Many shoulder conditions share similar symptoms, such as pain when lifting the arm, reaching overhead, sleeping on the affected side, or performing everyday activities. While these symptoms provide valuable clinical clues, they do **not** identify the exact cause on their own.

That's why a thorough history, physical examination, and—when clinically indicated—appropriate imaging are essential to determine the true source of the problem and guide the right treatment.
