Wonderful fellowship dinner organized by Pantai Hospital KL! It’s a bittersweet affair - delicious food and great company, enjoyed spending a relaxing evening with many friends and colleagues, but also sad to say goodbye to some very dear friends, Dr SS Gill and Dr Sng Kim Hock, who are both retiring, and to Miss Erica Lam, the longest serving CEO of PHKL who will be leaving us for greener pastures!

Dr Gill and Dr Sng, I will forever cherish the times we worked together! And to Erica, many thanks for your support over the years, and for steering the ship that is PHKL and for bringing us up to where we are today!

This weekend, Madam Mohana, at a young age of 92 yrs old, has officially become my oldest angioplasty patient! She presented recently with chest tightness on exertion suggestive of coronary heart blockage. In view of her age, I initially felt it was best to try a conservative approach with medications in the first instance, to alleviate her symptoms.

However despite medical therapy, her symptoms worsened and she was admitted to hospital with unstable angina this weekend. After much deliberation with the patient and family, we then decided to proceed with coronary angiography which revealed a 99% blockage in the top end and a further 70-80% blockage further down in one of her major heart arteries (LAD). Immediate coronary angioplasty with stenting was carried out successfully and the patient was in fact discharged home well the next day.

The case highlights an important take home message - with the latest innovations and medical technology, heart patients should NOT be denied treatment on basis of their age. Yes, as one becomes older and more frail the procedural risks do go up, but with careful planning and judicious care, these procedures can be carried out safely in selected individuals. Not all patients are the same - the risks and the potential benefits of improving survival or quality of life should be assessed carefully for every patient regardless of age!
