80 years old patient came to OPD on a stretcher, with acute deconditioning due to prolonged hospitalisation for 20 days in the hospital. She had renal impairment with a creatinine of 6.2 with anemia. She had a urinary catheter and a femoral catheter in-situ and was taking 12 medications including a diuretic. What did we do ?
Deprescription to 4 medications from 15 medications, urinary and femoral catheter removal, adequate hydration, iron infusion and nutrition. After 3 days, creatinine decreased to 0.9 from 6.2, appetite increased, she was able to walk without support and more importantly she was smiling !
