Aron mas mapaduol ang basic nga mga serbisyong panglawas sa matag pamilya sa atong komunidad, malipayon kaming nagpasabot nga sugod sa May 28, 2026 mahimo na kamong magpa check -up o regular nga magpa konsulta dinhi sa J-ANNE Maternity Clinic

🩺 Regular Consultation Schedule:
Monday – Saturday
⏰ 8:00 AM – 5:00 PM

✨ Mga Libreng Serbisyo pinaagi sa Yakap Konsulta sa Philhealth
✅ Naay libreng Konsulta ug Check-Up
✅ Naay libreng tambal para sa common nga mga sakit ug pipila ka maintenance medicines (subject to availability ug approved medicine/gamot list – hangtod 75 ka essential medicines), alang sa mga adunay resita gikan sa doktor, lakip ang:
• Tambal para sa hypertension / high blood
• Tambal para sa diabetes
• Antibiotics para sa common infections
• Tambal para sa asthma
• ORS (para sa dehydration)
• Vitamins ug uban pang maintenance medicines

✅ Laboratory ug diagnostic services nga i-request sa doktor
✅ Health education ug tambag aron malikayan ang sakit
✅ Continuous monitoring sa inyong health condition
✅ Referral ngadto sa mas taas nga healthcare facility kung gikinahanglan

📌 Palihug pagdala sa:
• Valid ID (preferably National ID)
• PhilHealth ID
• MDR (kung available)
📌 Para sa wala pa’y PhilHealth, mahimo gihapon mo magpakonsulta ug tabangan usab mo namo sa kinahanglanon nga impormasyon para sa PhilHealth registration.

🌟 Importante ang panglawas — ayaw hulata nga mosamot pa ang sakit bago magpakonsulta. Mas maayo ang sayong pag-atiman aron malikayan ang mas seryosong problema sa panglawas.

We are happy to share that J-Anne Maternity Clinic will soon become a PhilHealth YAKAP Clinic! 💚💛🏥

Soon, our clinic will continue its mission of caring for pregnant mothers and newborn babies while also expanding to provide accessible, quality and convenient primary healthcare services for everyone through the PhilHealth YAKAP/Konsulta Program. 👨‍👩‍👧‍👦💚💛

What is the YAKAP Program?
The YAKAP (Yaman ng Kalusugan Program) is a PhilHealth initiative that helps members gain easier access to essential healthcare services through accredited healthcare providers. These services may include consultations, regular check-ups, health assessments, selected laboratory services, medicines for common illnesses, and proper referrals when needed.

💙 Services/Benefits of Registering Under YAKAP/Konsulta
✔️ Free consultations and follow-up check-ups
✔️ Health screenings and assessment
✔️ Selected laboratory and diagnostic services covered by PhilHealth Konsulta such as:
• Complete Blood Count (CBC)
• Urinalysis
• Fecalysis
• Blood Sugar Testing
• Lipid Profile
• Chest X-ray
• Electrocardiogram (ECG)
• Other laboratory services based on PhilHealth coverage and doctor’s request
✔️ Medicines for common illnesses covered under PhilHealth Konsulta (subject to availability/Gamot List), which may include:
• Medicines for hypertension
• Medicines for diabetes
• Antibiotics for common infections
• Asthma medications
• Oral rehydration salts
• Vitamins and selected maintenance medicines

✔️ Health education and preventive care guidance
✔️ Continuous healthcare monitoring for mothers, babies, and the whole family
✔️ Proper referral to higher healthcare facilities when necessary

📝 How to Register

📱 Option 1: Through the eGovPH App/Philhealth Member Portal

1️⃣ Download the eGovPH App from the Google Play Store or App Store.
2️⃣ Create an account or log in.
3️⃣ Go to the PhilHealth/Konsulta section (PhilHealth Locate Accredited Health).
4️⃣ Search for and select J-ANNE Maternity Clinic as your Konsulta/YAKAP provider.
5️⃣ Confirm your registration.

🏥 Option 2: Walk-in Registration at the Clinic

1️⃣ Bring your PhilHealth Number and a valid ID.
2️⃣ Visit J-ANNE Maternity Clinic once YAKAP services officially become available.
3️⃣ Fill out the registration form.
4️⃣ Choose J-ANNE Maternity Clinic as your PhilHealth Konsulta/YAKAP provider.
5️⃣ Wait for confirmation and start enjoying your healthcare benefits. 💚💛
