- Performs a thorough oral examination
- Performs routine dental prophylaxis- Performs dental restoration- Performs simple dental extraction
- Prescribes medication when indicated
- Signs P.E. Form- Performs other dental emergencies such as drainage of abscess and treat gum diseases
- Records findings and treatment done on patient’s logbook
- Performs X-ray procedure if needed
- Coordinates with chief of clinics in order to deliver quality service
- Performs other functions as assigned by the Head Dentist form time to time

QUALIFICATIONS:
- Degree in Doctor of Dental Medicine
- Passed the Dentist Licensure Examination

📌 FINAL INSTRUCTIONS
✅ Call Time / Assembly Time
Ang tanan nga participants are advised to arrive on time sa venue for attendance and para ma claim ninyo ang stubs for your snacks (Free snacks for all the players na mag join)
✅ Proper Attire / Uniform
Please wear inyong assigned team uniform or appropriate sports attire para presentable ug komportable during the games.
✅ Bring Personal Essentials
Palihug lang ug dala sa inyong water tumbler, extra shirt/clothes, towel, ug uban pang personal necessities.
✅ Observe in sportsmanship throughout the event. Dula ta ug tarong ug enjoy lang sa atong activities.
✅ Follow the Schedule and Announcements
Kindly follow the official program, game schedules, and instructions gikan sa organizers para smooth ang dagan sa event.
✅ Health and Safety First

DUTIES AND RESPONSIBILITIES:

> Delivers service to both in and out patients by performing the following procedures:

Pulmonary function testing
Arterial blood gas analysis
Electrocardiography
Mechanical ventilation and weaning
> Monitors the operation of the area, ensuring good working conditions of equipment.

> Oversees the troubleshooting of equipment.

> Makes and follow-up request of repair equipment.

> Submits periodic section reports to the section head.

> Performs related functions as may be assigned by the immediate supervisor.

 Ensure that life support equipment, such as ventilators and feeding tubes, functions properly.
 Observes the patient’s heart rate, blood pressure, and respiration for signs of distress.
 Sees to it that every used equipment in the unit functions well.
 Assess and monitors the patient closely in order to identify subtle changes in a patient’s condition that warrant immediate intervention.
 Interprets, integrates, and responds to a wide array of clinical information.
 Ensures that the patient and/or family are well informed about the care that the patient is receiving, that the patient and family receive the necessary information to make informed and highly personal decisions about the patient’s care, and that the patient and family’s decisions are respected in the development of any treatment plan for the patient.

MEDICAL RECORDS CLERK

Duties and Responsibilities:
 Encodes admission records and discharged patients
 Arranges and assembles the medical records of discharged patient of the hospital.
 Verify listing from the list of vessel if the member/ dependents can avail of the privileges.
 Maintains surveillance of incomplete medical records and prepares reports of delinquent doctors.
 Incorporate loose reports/ sheets to respective charts
 Maintains and update the patient master index.
 Maintain proper filing of the medical records and periodically check files for misfiled records.
 Filed records and indexes according to the system
 Maintain a follow-up system for borrowed/ needed charts
 Retrieves requested records for follow up research and studies
 Collects and compiles daily census reports.
 Prepares computerized report of statistics
 Performs other related functions as may be assigned by the immediate supervisor
 Processes and submit Birth Certificate to City Hall
 Encodes letters and reports
 Review medical records to ensure that required reports and signatures have been included
 Processes death certificate
 In-charge in distributing and processing of dependent’s identification card
 Records and files family data
 Perform other related functions as may be assigned by the supervisor
