It is unfortunate that it has come to our knowledge that a previous client from a company has executed a falsified Medical Certificate bearing my name, signature and medical license including a PTR. This act is unlawful carries a penalty under the Revised Penal Code of the Philippines. Realized that,while your act of falsification suits your personal motives and interest, my licensed as Medical Doctor is put at risk. We warn the public that our medical clinic does not tolerate this kind of unlawful act.
