3. Last 3 Hemodialysis Treatment Sheet
4. Vaccination Record ( if available)
5. Latest Hepatitis profile (HBsag, Anti - HBs, Anti HCV)
6. Recent Laboratory Test (CBC with platelet, Creatinine, Sodium, Potassium, Blood typing )
7. Statement of Account from Previous Center /Hospital
PhilHealth Requirements:
1. MDR (Member Data Record)
2. PDD certificate (Philhealth Dialysis Database)
3. Updated Philhealth Usage 2023 from previous Dialysis Center
