📢 MAHALAGANG ANUNSIYO

Ikinalulungkot po naming ipaalam na ayon sa Department of Health (DOH), maaantala pa ang delivery ng suplay ng Pneumococcal Conjugate Vaccine (PCV).

❎ Dahil dito, pansamantala munang hindi maibibigay ang PCV 2nd dose at 3rd dose.

✅ Prayoridad munang mabigyan ng PCV ang mga batang tatanggap ng kanilang 1st dose.

Humihingi po kami ng paumanhin sa abalang dulot nito at taos-puso naming hinihiling ang inyong pang-unawa. Magbibigay po kami ng panibagong abiso sa oras na dumating na ang bagong suplay ng bakuna.

1. Magdala ng printed Philhealth Yakap e-prescription mula sa Health Center
2. Magpakita ng original and photocopy ng 1 valid ID ng pasyente:
• National ID (PhilSys)
• Passport
• UMID ID
• Driver’s License
• PRC ID
• SSS/GSIS ID
• PWD ID
• Senior Citizen ID
• TIN ID
3. Isulat ang contact number sa photocopy
4. Pirmahan ng 3 beses (wet signature o actual na lagda) ang photocopy ng valid id

❗ Paalala na kinakailangan na naka rehistro sa San Isidro Health Center bilang Yakap Provider ang pasyente para mabigyan ng E- Prescription

Available na po ang Oral Polio Vaccine sa ating health center. Para sa mga batang mayroong missed dose ng oral polio vaccine maaari na po silang ibalik sa health center sa araw ng bakuna simula bukas, Miyerkules. Dalhin lamang po ang vaccine card.
